Clinical Observation On The Treatment Of Ankylosing Spondylitis Based On The "yi And Gui Homogenous" Theory Of Chinese Herbal Formulae For Liver And Kidney Deficiency

Objective:In this study,we observed the improvement of clinical symptoms and indexes before and after the application of Rouji Tongdu Decoction combined with western drugs Sulfasalazine Eteric-coated Tablats(SASP)and Imrecoxib Tablets in ankylosing spondylitis with liver and kidney deficiency(AS)based on the theory of "yi and gui homogenous".To evaluate the clinical efficacy and safety of Rouji Tongdu Decoction combined with SASP+Imrecoxib Tablets in the treatment of hepatorenal deficiency AS based on the theory of "yi and gui homogenous",and to further discuss the clinical efficacy and safety of Rouji Tongdu Decoction combined with SASP+Imrecoxib Tablets in the treatment of hepatorenal deficiency AS based on the theory of "yi and gui homogenous".We further investigate the superiority of the "yi and gui homogenous" theory of Chinese herbal formulas in guiding the clinical treatment of ankylosing spondylitis with liver and kidney deficiency,and provide an efficient and safe new approach for the combined treatment of ankylosing spondylitis with Chinese and Western medicine.Methods:Sixty-four patients who met the enrollment requirements were selected from patients attending the rheumatology outpatient clinics and wards of the Affiliated Hospital of Shandong University of Traditional Chinese Medicine-Yantai Hospital of Traditional Chinese Medicine between October 2019 and June 2021.The enrolled patients were randomly divided into the treatment group and the control group.The two groups were treated conventionally with oral SASP and Imrecoxib Tablets,and the treatment group was treated with a combination of Rouji Tongdu Decoction on top of Western medicine.We assessed the degree of improvement of TCM symptoms and clinical efficacy and analyzed the statistical significance by recording and analyzing the objective changes of CRP,ESR,spinal mobility(thoracic mobility,occipito-wall distance,finger-ground distance,Schober’s test),BASDAI,BASFI scores and TCM syndrome score in patients before and after treatment.Results:1.A total of 41 males and 23 females were included in the study,with a mean age of 35.380±10.573 years in the treatment group and 34.630±10.044 years in the control group.The mean duration of disease was(5.200±3.410)years in the treatment group and(4.920 ± 3.610)years in the control group.Statistical analysis of patients’ basic data(gender,age,disease duration),bilateral sacroiliac joint CT grading and observation indexes before treatment(ESR,CRP,thoracic mobility,occipito-wall distance,finger-ground distance,Schober’s test,BASDAI,BASFI,TCM evidence score)were not statistically significant(P>0.05).2.After 12 weeks and 24 weeks of treatment,the ESR and CRP inflammation indexes gradually decreased in both groups,and after 24 weeks of treatment,it was found that the effect of combining Western medicine with Tonic Void Warming and Tongluo Tang was superior,with statistical significance(P<0.05).3.After treatment,the signs and symptoms of both groups improved,and the treatment with Rouji Tongdu Tang combined with western medicine(SASP +ereciclib tablets)was more effective in improving the signs and symptoms of patients than the treatment with western medicine alone,and the difference was statistically significant(P<0.05).4.After treatment,the BASDAI,BASFI,and TCM evidence points were reduced in both groups,among which the effect of combining Western medicine with Rouji Tongdu Decoction was better,and the difference was statistically significant(P<0.05).5.As for the efficacy of Chinese medical evidence after treatment,the total effective rate of combining Western medicine with Tonic Deficiency and Wen Du Tong Luo Tang was 93.75%(30/32),and the total effective rate of Western medicine alone was 87.50%(28/32);In terms of clinical efficacy,the total effective rate of the treatment with the combination of Western medicine was93.75%(30/32)and the total effective rate of the treatment with Western medicine alone was 68.75%(22/32).After 24 weeks,both groups showed good TCM symptom efficacy and clinical efficacy,and the TCM symptom efficacy and clinical efficacy of Rouji Tongdu Decoction combination of Western drugs(SASP+Imrecoxib Tablets)were more prominent,and the difference was statistically significant(P<0.05).6.Four patients in the control group(SASP +Imrecoxib Tablets)experienced mild nausea with loss of appetite after taking the drug,which resolved on its own.The difference in treatment safety between the two groups was not statistically significant(P < 0.05).Conclusion:Rouji Tongdu Decoction combined with western medicine(SASP+Imrecoxib Tablets)significantly reduced ESR,CRP,BASDAI score,BASFI score,and TCM evidence score,and improved the patients’ physical signs with better efficiency than treatment with SASP+Imrecoxib Tablets based on the "yi and gui homogenous" theory.Both treatment groups have a certain degree of safety.Rouji Tongdu Decoction combined with western medicine(SASP +ericiclib tablets)is more effective in the treatment of ankylosing spondylitis with liver and kidney deficiency.
