To Explore The Perioperative Clinical Efficacy Of Dong Needle + Buccal Needle For TKA Based On ERAS Concept

Object:Based on the ERAS management concept on the perioperative period of total knee arthroplasty,the clinical effect of Dong acupuncture + buccal acupuncture on pain in patients after total knee arthroplasty,as well as its knee joint activity score and safety,are discussed for the improvement of acupuncture treatment.Provide a reliable basis for pain after total knee arthroplasty.Methods:A total of 90 patients who were diagnosed with knee osteoarthritis in the Orthopedics Department of Linyi Traditional Chinese Medicine Hospital from November 2020 to October 2021 and were determined to need unilateral total knee arthroplasty were randomly selected and randomly divided into groups A,B Groups,C,and A were managed by routine perioperative period;group B was managed by enhanced recovery surgery(ERAS)concept,multimodal analgesia:patient-controlled intravenous analgesia(PCIA)+ femoral nerve anesthesia block + 2 hours before surgery,0.1 g of erecoxib were given 6 h after operation,and then continued for 7 days,2 times a day,0.1 g once;group C was treated with Dong acupuncture + buccal acupuncture instead of erecoxib on the basis of group B,2 h before operation,Dong acupuncture and buccal acupuncture were applied 6 hours after the operation,followed by continuous acupuncture for7 days,once a day for 40 minutes.By observing and comparing the pain visual analog(VAS)score,the number of times of patient-controlled intravenous analgesia(PCIA)pressing,the American Hospital for Special Surgery(HSS)score of knee joint,and the incidence of postoperative adverse events in the three groups of patients,to explore the Dong acupuncture + buccal For postoperative clinical efficacy.Results:(1)There was no significant difference in the VAS pain scores between the three groups before operation(P>0.05).The VAS score of the latter3 days was better than that of the group B,and the VAS score of the 5th and10 th days after the operation was not significantly different from that of the group B(P>0.05).(2)Comparison of the compression times of the patient-controlled intravenous analgesia(PCIA)among the three groups,the compression times of group C were significantly less than that of groups A and B within 24 hours and 24-48 hours after operation,and mainly concentrated in 0-1 times;According to statistics,the number of times of patient-controlled intravenous analgesia(PCIA)compressions from large to small: group A > group B > group C(P< 0.05).(3)There was no significant difference in the HSS scores of the knee joints between the three groups before surgery(P>0.05).The HSS scores of the knee joints in group C at 7d and 14 d after surgery were higher than those in groups A and B(P<0.05).The knee joint HSS score at 7d and 14 d after operation was higher than that in group A(P<0.05).(4)Among the three groups of postoperative adverse reactions,the incidence of group C was less than that of groups A and B(P<0.05),and there was no significant difference between group A and group B in the incidence of adverse reactions(P>0.05).Conclusion :(1)The postoperative analgesia effect of ERAS concept management during the perioperative period was better than that of conventional perioperative operation;the short-term analgesic effect of Dong acupuncture + buccal acupuncture during the perioperative period based on the ERAS concept was better than that of erecoxib.(2)The ERAS concept management during the perioperative period is more obvious than the conventional perioperative operation for the improvement of postoperative knee joint function.Based on the ERAS concept management during the perioperative period,the application of Dong acupuncture + buccal acupuncture can accelerate the improvement of postoperative patients.Knee joint functional activities.(3)Compared with the conventional perioperative operation,there was no significant difference in the occurrence of postoperative gastrointestinal adverse reactions in patients managed by the simple western medicine scheme managed by the ERAS concept during the perioperative period.Based on the ERAS concept management during the perioperative period,Dong acupuncture + buccal acupuncture was applied Can relatively reduce the occurrence of gastrointestinal adverse events in postoperative patients...
