Objective: To summarize the distribution characteristics of pathogenic bacteria,risk factors of complicating with pulmonary infection and the Traditional Chinese Medicine(TCM)prescription to provide reference for the prevention and treatment of patients according to the results of etiological examination,the basic diseases and selected Chinese medicine prescriptions by retrospectively analyzing the clinical data of elderly patients with lung cancer complicated with pulmonary infection.Methods:The clinical data of diagnosed patient in the department of Geriatrics and Pulmonary Diseases of the Affiliated Hospital of Shandong University of TCM in the recent 10 years from January 1,2012 to December 31,2021 was collected.Then SPSS25.0 was used to analyze the distribution of pathogenic bacteria and risk factors associated with pulmonary infection.The software of TCM inheritance assistance platform V2.5 was used to analyze the medication frequency,efficacy classification,flavor and meridian tropism and pharmaceutic rule of the selected prescriptions.Results:1 Distribution characteristics of pathogenic bacteria in elderly patients with lung cancer complicated with pulmonary infectionA total of 334 elderly patients with lung cancer were selected,of which 205 cases were complicated with pulmonary infection,that is,the incidence of elderly lung cancer complicated with pulmonary infection was 61.38%.And 141 strains of pathogenic bacteria were isolated from sputum culture,including gram-negative bacteria(74),gram-positive bacteria(42)and fungi(25).Gram-negative bacteria mainly included Acinetobacter Baumannii(10.64%),Klebsiella Pneumoniae(10.64%),Stenotrophomonas Maltophilia(9.22%)and Pseudomonas Aeruginosa(7.80%).While gram-positive bacteria mainly included Staphylococcus Aureus(14.18%)and Enterococcus Faecium(8.51%).And Candida Albicans(6.38%)of the fungi were more common.2 Risk factors of pulmonary infection in elderly patients with lung cancer334 cases of elderly patients with lung cancer,the incidence of secondary pulmonary infection was 61.38%.The related factors including TNM stage,smoking,lung cancer surgery,chemoradiotherapy,basis respiratory system disease,cerebrovascular disease,type2 diabetes,hypoalbuminemia,electrolyte disorders were deemed to the risk factor of elderly patients with lung cancer complicated with pulmonary infection.3 Analyzing the features of TCM prescription in elderly patients with lung cancer complicated with pulmonary infection based on the TCM inheritance auxiliary platformAmong 205 elderly patients with lung cancer complicated with pulmonary infection,193 patients used the herbal decoction with a utilization rate of 94.15%,which involving211 herbs.According to the analysis results,the top 20 high frequency of herbs were separately as follows: Gancao,Huangqi,Beimu,Baizhu,Chenpi,Fuling,Maidong,Dangshen,Huangqin,Kuxingren,Wuweizi,Qianhu,Banxia,Jiegeng,Jinyinhua Shanyao,Chuanxiong,Danshen,Pipaye and Mahuang.In the distribution of drug efficacy,the top 6categories were Buxu,Qingre,Huatan zhike pingchuan,Lishui shenshi,Huoxue huayu and Liqi.In terms of syndrome type distribution,the syndrome of deficiency of Qi and Yin,phlegm and heat blocking lung,deficiency of lung and spleen Qi and blood stasis blocking lung were the main ones.In the analysis of four qi and five tastes,the frequency of warm,cold,sweet and bitter was higher.The top 5 meridian categories were lung meridian,spleen meridian,stomach meridian,heart meridian and liver meridian.Furthermore,four new prescriptions summarized mainly have the effects of regulating qi and reducing phlegm,invigorating spleen and reducing dampness,clearing heat and nourishing Yin by the analyze.Conclusion:1 The distribution of pathogenic bacteria in elderly lung cancer complicated with pulmonary infection is diverse.It is necessary to strengthen the culture of pathogenic bacteria and the examination of drug sensitivity and rationally select antibacterial drugs so as to treat the symptoms and reduce the generation of pathogenic bacteria resistance in clinical application of antibiotics.2 The TNM stage of lung cancer,smoking,chemoradiotherapy,lung cancer surgery,combined with basic respiratory diseases,cerebrovascular diseases,type 2 diabetes,hypoproteinemia,electrolyte disorders,etc.,are the risk factors of elderly lung cancer combined with pulmonary infection and clinical attention should be paid to the prevention and treatment of pulmonary infection in lung cancer patients with these factors.3 The pathogenesises of this disease are mainly summarized as deficiency of Qi and Yin and phlegm and heat blocking lung by the TCM inheritance assistance platform.In terms of treatment,the main principles are including invigorating Qi and nourishing Yin and clearing heat and reducing phlegm.What’s more,and four new prescriptions have been obtained which can provide new ideas,data reference,theoretical support and the establishment of new prescription for the clinical application of TCM in the treatment of elderly patients with lung cancer complicated with pulmonary infection.
