| Objective: To explore the clinical effect of modified Bazhen decoction on hidden blood loss after THA to provide reference for clinical treatment Methods: According to the case inclusion and exclusion criteria,66 patients who underwent primary unilateral THA surgery for femoral head necrosis in the Department of Joint Orthopedics,Affiliated Hospital of Shandong University of Traditional Chinese Medicine from December 2020 to December 2021 were selected.They were randomly divided into observation group and control group,with 33 cases in each group.The control group was given routine perioperative treatment,and the observation group was given Modified Bazhen Decoction on the basis of the control group,taking one dose a day,from 2 days before surgery to the 5th day after surgery,and not on the day of surgery.Venous blood was drawn from the observation group and the control group on the 2nd day before operation and on the 1st,3rd,and 5th days after the operation,respectively,and the hemoglobin,hematocrit and D-dimer were recorded.The intraoperative blood loss,postoperative drainage volume,and the Harris score of the hip joint 2 days before and 45 days after the operation were calculated.Using spss21.0 software to analyze the data,P<0.05 means the difference is statistically significant.Results: In this study,2 patients in the observation group dropped out,31 cases were actually completed,and 3 patients in the control group dropped out,30 cases were actually completed,and a total of 61 cases were completed.1.General information: There was no significant difference in general information(gender,age,height,weight and BMI)between the two groups(P > 0.05).2.Laboratory indicators: There was no significant difference between the groups on the 2nd day before operation,the 1st and 3rd day after operation(P>0.05).On the 5th day after operation,the hemoglobin and hematocrit of the observation group were significantly higher than those of the control group,and the D-dimer was significantly lower than that of the control group,and the differences were statistically significant(P<0.05).Compared within the group,the hemoglobin and hematocrit in the two groups decreased from the 2nd day before the operation to the 3rd day after the operation,and increased from the 3rd day after the operation to the 5th day after the operation,and the difference was statistically significant(P<0.05);D-dimer in both groups increased from 2 days before operation to 1st day after operation,and decreased from 1st day after operation to 5th day after operation,the differences were statistically significant(P<0.05).3.Blood loss: There was no significant difference in the estimated blood volume between the two groups before treatment(P>0.05).After treatment,the hidden blood loss and total blood loss in the observation group were significantly lower than those in the control group,and the difference was statistically significant(P<0.05).4.TCM syndrome score: Before treatment,there was no statistically significant difference in syndrome scores between the two groups(P >0.05),and after treatment,the observation group was lower than the control group,the difference was statistically significant(P < 0.05).Intra-group comparison,the syndrome score of 2 groups after treatment was significantly lower than that before treatment,with statistical significance(P < 0.05).5.Comparison of TCM syndrome efficacy: The total effective rate of the observation group was 93.55%,and the total effective rate of the control group was 83.33%,with statistically significant difference between the two groups(P < 0.05).6.Harris score: There was no significant difference in Harris score between the observation group and the control group 2 days before surgery(P > 0.05),and Harris score in the observation group was significantly higher than that in the control group 45 days after surgery(P < 0.05).Intra-group comparison,Harris score of the two groups increased 45 days after surgery compared with 2 days before surgery,with statistical significance(P < 0.05).Conclusion: Modified Bazhen Decoction can increase postoperative hemoglobin and hematocrit levels,reduce D-dimer levels;reduce postoperative hidden blood loss and total blood loss;reduce TCM syndrome scores;improve effective treatment rate;increase Hip Harris score.Modified Bazhen Decoction can help reduce the risk of anemia after THA and promote rapid recovery of patients. |
