| Objective:The purpose of this subject is to explore the current situation of Traditional Chinese Medicine(TCM)nursing technology evaluation which used literature analysis,semi-structured interview,Delphi method and Analytic hierarchy process(AHP),so as to construct an index system of TCM nursing technology evaluation in accordance with the characteristics of TCM and provide objective reference for TCM nursing technology evaluation.Methods:1.This study is aim to understand the development status of health technology assessment at home and abroad and the characteristics of TCM nursing technology by consulting the relevant literature of TCM nursing technology and health technology assessment.2.Through semi-structured interviews,face-to-face in-depth interviews were conducted on clinical TCM medical and nursing staff,in order to fully understand the characteristics of TCM nursing technology.Then,based on the results,we preliminarily constructed the framework of TCM nursing technology evaluation index system.3.Two rounds of expert letter inquiries were conducted by Delphi method,and the statistical indexes such as positive degree,authority degree and coordination degree of 19 experts were calculated.Then the analytic hierarchy process was used to establish the judgment matrix,calculate the weight of all levels of indexes and check the consistency,In the end,the results formed the final TCM nursing technology evaluation index system.Results:1.In this study,three themes were formed through semi-structured interviews with 8 clinical TCM medical and nursing staff:the current situation of TCM nursing technology evaluation,the necessity of TCM nursing technology evaluation,and the screening of TCM nursing technology evaluation indexes.2.TCM nursing technology evaluation index system was formed by 6 first-level indexes which included maturity,technical characteristics,simplicity,safety,effectiveness,social and ethical adaptability,13 second-level indexes and 34 third-level indexes.3.A total of 19 experts participated in the Delphi expert letter inquiry in this study.Through the qualitative analysis of the general data of each expert and the quantitative analysis of the degree of expert authority,it was shown that the participating experts had good representativeness and authority.The recovery rates of the two rounds of letter inquiry are 95%and 100%respectively which meant the experts were more active;the degree of authority(Cr)was 0.941 and the authority of expert opinions was high.The expert consultation coefficients of the two rounds of Delphi expert letters were 0.321 and 0.341 respectively,and the result of significance test was statistically significant(P<0.05),and the degree of expert coordination was advanced.After the second round of Delphi expert letter inquiry,the assigned average of all indexes was more than 3.5.The coefficient of variation was less than 0.25.Meanwhile,at least one of the cutoff values of mean,coefficient of variation and full score rate was the best.Finally,a TCM nursing technology evaluation index system was formed,which included 6 first-level indexes,13 second-level indexes and 34 third-level indexes.4.The weights of the first-level indexes were maturity(0.1649),technical characteristics(0.1039),convenience(0.0468),safety(0.3662),effectiveness(0.2495),social and ethical adaptability(0.0687).Among the second-level indexes,the top three combined weights were adverse event severity(0.2747),satisfaction(0.1481)and application(0.1319).Then among the three-level indexes,the top five combined weights were patient physical and mental injury(0.1831),patient satisfaction(0.1185),patient economic factor(0.0916),technical operation standard and procedure(0.0840)and technical operation factor(0.0732),and the CR value of each level index was less than 0.1.Conclusion:1.The method of this study was scientific and rigorous,the index was true and complete,and the experts who participated in Delphi expert consultation had good enthusiasm,high degree of authority and coordination,which made the prediction accuracy higher and the consultation results more reliable.2.The established evaluation index system of traditional Chinese medicine nursing technology fully took into account the characteristics of TCM and the actual situation of TCM nursing technology in the development of traditional Chinese medicine.The evaluation index system was constructed from six aspects:maturity,technical characteristics,simplicity,safety,effectiveness,social and ethical adaptability,and it was with wide coverage,reasonable structure and complete content.3.On the basis of constructing the index system,this study used the analytic hierarchy process to calculate the weight of the index,ensured the science and rationality of the calculation results,and objectively showed the importance of all levels of indexes.It provides a reference basis for the access,application and promotion of TCM nursing technology. |
