Objective:To observe the therapeutic effect of Tongqiao Huoxue decoction on acute ischemic stroke and its effect on ET-1 and CGRP through clinical and animal experiments,and to explore the protective effect and possible mechanism of Tongqiao Huoxue decoction on acute ischemic stroke from the point of view of vascular endothelial protection,so as to provide clinical ideas and experimental basis for TCM treatment of acute ischemic stroke.Methods1.Clinical study: A total of 60 patients with acute ischemic stroke in the department of neurology in our hospital were randomly divided into two groups:routine western medicine group(n=30)and routine western medicine +Tongqiao Huoxue decoction group(n=30).The treatment period was 2weeks.The related neurological function scores(NIHSS and m RS scores)were performed on the 1st and 14 th day of ischemic stroke in the two groups,and blood samples were collected to detect the expression of serum ET-I and CGRP before and after treatment by ELISA detection and analysis and comparison.2.Experimental study: in this study,50 healthy male SD rats were used as subjects,of which 10 rats were used as sham operation group,and the other 40 rats were used to establish MCAO animal model.The successful rats were randomly divided into 4 groups: model group and Tongqiao Huoxue decoction high,middle and low dose groups,8 rats in each group(excluding death and substandard models).The rats in the sham operation group and the model group were given the same volume of normal saline,and the rats in the Tongqiao Huoxue decoction group were given intragastric administration at high,middle and low doses for 14 days,once a day,respectively.The neurological function of the rats in each group was measured on the 14 th day,and the rats were killed 2hours after the last administration.The contents of ET-1 and CGRP in rat brain tissue were detected by RT-q PCR method,and the infarct was observed by TTC staining.Results1.The results of clinical experiment showed that there was no significant difference in neurological function score and serum ET-1 and CGRP expression between the two groups on the first day of treatment.After 2 weeks of treatment,there were significant differences in NIHSS score and m RS score.Serum ET-1expression decreased and serum CGRP expression increased.It shows that the treatment schemes of the two groups can improve the neurological function of the patients and improve the vasomotor function of blood vessels.There were significant differences in neurological function score and serum ET-1 and CGRP expression between the routine western medicine + Tongqiao Huoxue decoction group and the routine western medicine group(P<0.05),indicating that the routine western medicine + Tongqiao Huoxue decoction group could improve the neurological function and vasomotor function of the patients better.2.The results of animal experiment showed that compared with the sham operation group,the expression level of ET-1 in brain tissue of model group and Tongqiao Huoxue decoction group increased(P<0.05),while the expression level of CGRP decreased(P<0.05).Compared with the model group,the expression level of ET-1 in brain tissue of rats in each dose group of Tongqiao Huoxue decoction decreased in different degrees(P<0.05),while the expression level of CGRP increased in different degrees(P<0.05).Compared with the high dose group of Tongqiao Huoxue decoction,the expression of ET-1 increased in the middle and low dose groups of Tongqiao Huoxue decoction,while the expression of CGRP decreased in the middle and low dose groups,which indicated that the curative effect of high dose group was better than that of middle and low dose groups.Conclusions:1.Clinical study found that the expression of serum ET-1 increased and the expression of CGRP decreased in patients with acute ischemic stroke,while the treatment group of conventional western medicine plus Tongqiao Huoxue decoction decreased the expression of serum ET-1 and upregulated the expression of CGRP more effectively than that of simple western medicine group,and improved the neurological deficit of patients with acute ischemic stroke more obviously.2.Animal studies showed that the expression of ET-1 increased and the expression of CGRP decreased in the brain tissue of MCAO rats.Compared with the model group,the high,middle and low dose groups of Tongqiao Huoxue decoction could improve the expression of ET-1 and CGRP,and the effect of high dose group was more significant.3.Tongqiao Huoxue decoction may protect vascular endothelial function by affecting the relaxing and contractile effect of vascular endothelial factor in the treatment of acute ischemic stroke.
