The Key Of Pension Institution Based On Structure-Process-Result Theory Construction Of Nursing Quality Index System

Objective: The key care quality evaluation index system of elderly care institutions was constructed,including index name,definition,significance,calculation formula,data collection method,calculation formula,etc.To provide reference for evaluating the nursing service level of various domestic nursing institutions.Methods: Guided by the structure-process-results(SPO)theoretical model,this study adopted literature review method and focus group discussion method to develop the first draft of key care evaluation indicators for elderly care institutions,and completed the expert letter questionnaire.After strictly selecting 20 experts,Delphi expert letter consultation method was used to carry out 2 rounds of correspondence consultation.The interval between two rounds of correspondence is 25 days.Epidata3.1 was used to input the obtained data,and SPSS20.0 and Ya AHP11.0 software were used to conduct descriptive statistical analysis,expert positive coefficient,expert opinion authority,expert opinion coordination degree and trend analysis.Delphi method is used to calculate the weight of each index in the index system to ensure the rationality and stability of the weight of each index,and finally establish a scientific and reliable evaluation index system.Results: Eight indicators were preliminarily established in this study.It contains three dimensions: importance of index,rationality of index formula and operability of index.The effective recovery rates of the two rounds of questionnaires were 100%.In the first round of letter consultation,13 experts proposed suggestions for modification.After the first round of expert letter consultation,2 indicators were deleted,3indicators were modified and 3 indicators were added.In the second round of correspondence,four experts provided their views.Kendal harmony coefficients of the importance of indicators,reasonableness of indicator formula and operability of indicators in the two rounds of expert letter consultation were 0.547,0.301,0.519 and 0.679,0.682,0.657,respectively.Finally,the key care evaluation index system of pension institutions has 9 evaluation contents,including: Proportion of nursing staff to the elderly,rate of body injury caused by physical restraint,incidence of stress injury,incidence of falling pneumonia,choking/aspiration,incidence of falling or falling bed(chair),incidence of medication error,incidence of unplanned extubation,incidence of indwelling catheter related urinary tract infection.Conclusion: Under the guidance of structure-process-results(SPO)theoretical model,this study constructed 9 key care quality evaluation indicators for elderly care institutions: Proportion of nursing staff to the elderly(weight 0.116),tissue injury rate caused by physical restraint(weight 0.103),incidence of stress injury(weight 0.113),incidence of falling pneumonia(weight 0.111),incidence of choking/aspiration(weight 0.111),incidence of falling or falling into bed(chair)(weight0.113),drug use The incidence of errors(weight 0.113),unplanned extubation(weight 0.110),and catheter-related urinary tract infection(weight 0.111).The index definition,calculation formula and data collection method are established.
