Study On Serum Myocardial Enzymes In Infants With Rotavirus Enteritis

Objective:By analyzing the characteristics of myocardial zymogram of rotavirus enteritis,the relationship between serum myocardial zymogram of infants and young children and rotavirus enteritis was discussed,so as to provide objective basis for diagnosis and treatment of rotavirus enteritis.Methods:The clinical data and serum Creatine kinase(CK),Creatine kinase isoenzyme(CK-MB),Hydroxybutyrate dehydrogenase(HBDH),Lactate dehydrogenase(LDH),and Aspartate aminotransfrease(AST)of moderate and severe rotavirus enteritis and non-rotavirus enteritis hospitalized in Yan’an University Affiliated Hospital from January 1,2018 to December 31,2021 and tested rotavirus antigen were collected.Gender,daily diarrhea frequency,days of onset,vomiting and fever were compared between children with severe and medium rotavirus enteritis.The serum levels of CK,CK-MB,HBDH,LDH,and AST were compared between children with rotavirus enteritis and non-rotavirus enteritis.Serum levels of CK,CK-MB,HBDH,LDH,and AST were compared between children with severe and medium rotavirus enteritis.The serum myocardial enzymes of children with severe rotavirus enteritis were analyzed by receiver operating curve(ROC).Results:1.In this study,the seasonal distribution of hospitalized children with rotavirus enteritis in the affiliated hospital of Yan’an University is 14 cases(9.09%)in spring,6 cases(3.90%)in summer,6 cases(3.90%)in autumn,and 128 cases(83.11%)in winter.2.In this study,boys were predominant in the moderate rotavirus enteritis group(61.4%),while girls were predominant in the severe rotavirus enteritis group(65%),the difference being statistically significant(χ~2=8.326,P<0.05).3.The number of children with diarrhea<5 times a day in moderate rotavirus enteritis group was higher than that in the severe rotavirus enteritis group,the number of children with diarrhea≥5 times a day was higher than that in the severe rotavirus enteritis group,the difference was statistically significant(χ~2=10.428,P<0.05).There were no statistically significant differences in onset days(<3,≥3),vomiting(no/yes)and fever(no/yes)between severe rotavirus enteritis group and moderate rotavirus enteritis group(P>0.05).4.The levels of CK,CK-MB,HBDH,and AST in the rotavirus group were higher than those in the non-rotavirus group(P<0.001);the LDH level in the rotavirus group was higher than that in the non-rotavirus group(P<0.05).5.Serum levels of CK,CK-MB,and LDH in the severe rotavirus enteritis group were higher than those in the medium rotavirus enteritis group,and the difference was statistically significant(P<0.001).Serum HBDH level in the severe rotavirus enteritis group was higher than that in medium rotavirus enteritis group,and the difference was statistically significant(P<0.05).No statistical significance(P>0.05).6.The ROC curve analysis results of severe rotavirus enteritis showed that the cardiac enzymes CK and CK-MB were of great significance for interpretation.The area under the curve,P-value,Cut-Off value,Youden index,sensitivity and specificity were 0.780,0.000,186.50,0.497,55.0%,94.7%,0.860,0.000,134.65,0.667,82.5%,84.2%;myocardial enzymes HBDH and LDH have little interpretation significance,and their area under the curve,P-value,Cut-Off value,Youden index,sensitivity and specificity were 0.635,0.011,335.50,0.210,35.0%,86.0%,0.700,0.000,307.50,0.375,70.0,67.5%,respectively.The combined ROC curve analysis of these four indicators showed that the area under the curve was 0.867,P=0.000,the Youden index was 0.612,the sensitivity was 70.0%,and the specificity was 91.2%.Conclusion:1.In this study,the onset season of hospitalized children with moderate and severe rotavirus enteritis in Yan’an University Affiliated Hospital is mainly distributed in winter.Among them,the hospitalized children with moderate rotavirus enteritis are mainly boys(61.4%),and the hospitalized children with severe rotavirus enteritis are mainly girls(65%).3.With the increase of daily diarrhea,the severity of rotavirus enteritis also increased;In addition,the indexes of myocardial enzyme spectrum were increased in rotavirus enteritis,and the levels of serum CK,CK-MB,HBDH,and LDH were significantly increased with the increase of disease severity.It is suggested that the changes of myocardial enzyme level have clinical guiding significance for the early recognition,treatment and prevention of rotavirus enteritis.3.CK and CK-MB in serum myocardial enzymes have great significance in the determination of severe rotavirus enteritis,and their optimal cut-off values have certain clinical reference significance for judging the severity of rotavirus enteritis.Combined analysis of myocardial enzyme profile can improve the interpretation significance of myocardial enzyme profile in severe rotavirus enteritis.
