Medical reform supervision is an important institutional arrangement that accompanies the reform of the medical and health system,aiming to promote the implementation of reform measures and promote the deepening of medical reform.It has been operating in practice for more than ten years and has played an important role in improving the effectiveness of medical reform policies.At present,medical reform has entered a deep-water area,touching deep-seated contradictions to slow down this reform process,and medical reform supervision can help medical reform overcome difficulties through resource coordination and strengthened accountability to a certain extent.At present,the theoretical research based on medical reform supervision is far behind practice,and it is difficult to improve its own efficiency with the help of complete institutional system and operation mechanism research.Based on this,this paper takes the medical reform supervision as the research object,and exposes the actual state of the medical reform supervision through the two angles of institutional text combing and practical operation analysis.This paper attempts to combine organizational theory analysis with the medical reform supervision system,take the organizational analysis perspective in policy implementation as the starting point,sort out and summarize the relevant concepts,and use the text analysis method,semi-structured interview method and empirical research method to analyze the operation mode of the current medical reform supervision system.On the basis of research construction,the core content of this paper is that the third chapter analyzes the medical reform supervision system system and its operation mechanism through text analysis and data collection,describes the evolution of the medical reform supervision system and its composition from the perspective of historical changes,constructs the medical reform supervision system system from the three dimensions of organizational structure,operation process,and methods,and lays the institutional foundation for the subsequent research;the fourth chapter takes Hubei Province and its subordinate Xiangyang City as the empirical research object,through in-depth excavation of interview materials.Analyze the dilemma of the medical reform supervision system under multiple logics.Through the construction of multiple logical frameworks,the implementation dilemma of medical reform supervision is deeply analyzed from the three levels of technical governance,imbalance of power and responsibility,and coordination failure;the fifth chapter focuses on the use of the organizational theory analysis framework of policy implementation to discuss the dilemma of the medical reform supervision system,and analyzes the logic of the dilemma from the three perspectives of organization internal,organizational relationship and organizational environment,forms a good link,and comprehensively outlines the implementation of the medical reform supervision system.Finally,the sixth chapter summarizes and sorts out the above research findings,and provides strategic choices for solving the current dilemma of the effectiveness of China’s medical reform supervision from a practical point of view. |
