Objective:This research intends to adopt low frequency amplitude and morphological measurement method based on voxel, using the technology of resting state functional magnetic resonance imaging data acquisition,a deep study of adolescents with depression with somatic pain neurons in the brain functional activity, research the specificity of the resting state function abnormal brain changes, and investigate the neuroimaging mechanism of depression with body pain,to provide scientific basis for clinical prevention and treatment.Methods:49 adolescents who met the Diagnostic criteria for depression in the Diagnostic manual of mental disorders,5thedition(DSM-5)were collected as subjects. The age range was 13-18 years old,and all cases were admitted to the psychological consultation out-patient Department of Affiliated Hospital of Zunyi Medical University.Among them,28patients with depression without body pain were treated as Case group 1,and 21 patients with body pain were treated as Case group 2.The research method was case control.23healthy adolescents whose general information was consistent with the Case group, such as gender, age, education level and family background, were recruited as the control group.General data were collected for all cases, followed by Beck Depression Inventory(BDI),Hamilton Depression Rating Scale(HAMD-24),Visual Analogue Scale(VAS)and Numerical Rating Scale(NRS)were used for functional magnetic resonance(fMRI)scanning.DPARSF software and SPM12 running on MATLAB platform were used to process functional magnetic resonance imaging of all groups, and ALFF data were obtained. After that, the images of Case group 1、group 2 and control group were compared and analyzed to find out the brain regions with statistical differences, and then the gray matter VBM correlation analysis was carried out using this as a template Mask. Results:There was no significant difference in the general data of the three groups(P>0.05).The HAMD-24 and BDI scores of the Case group were analyzed, and the scores of the Case group 2 were significantly higher than that of the Case group 1,with statistical significance(P<0.05).Functional magnetic resonance imaging analysis of the Case group showed that ALFF value increased in brain regions including the right medial superior frontal gyrus and the left insular, and decreased in brain regions including the left dorsolateral superior frontal gyrus and the left medial superior frontal gyrus, the difference was statistically significant(P<0.05,FDR correction). The results showed that ALFF value of left insula was positively correlated with HAMD-24 score and VAS score in Case group2.In Case group 2 Mask the template based image gray matter volume calculation,it is concluded that the Case group 2 on the left side of the medial frontal last time, the last time the right medial frontal and right dorsolateral frontal gyrus, left insula gray matter volume was reduced, the left dorsolateral frontal gyrus gray matter volume was increased, statistically significant difference(P<0.05,FDR correction). Conclusion:Adolescents with depression and physical pain have abnormal brain activity and structure, mainly involving brain regions including insular and superior frontal gyrus, and gray matter volume changes in some cases. The abnormal activity of the left insular is positively correlated with the severity of depression and pain, which has significance for the analysis and evaluation of the disease.
