Clinical Study Of Traditional Chinese Medicine Fumigation Combined With Intense Pulsed Light In The Treatment Of Meibomian Gland Dysfunction Dry Eye

Objective:To observe the clinical efficacy of traditional Chinese medicine fumigation combined with intense pulsed light in the treatment of meibomian gland dysfunction dry eye.Methods:From January 2021 to October 2021,108 cases(216 eyes)of meibomian gland dysfunction dry eye patients with wind-heat syndrome who met the inclusion criteria in the Department of Ophthalmology of the Second Affiliated Hospital of Fujian University of Traditional Chinese Medicine were collected,and divided according to the random number table.There were 36 cases with 72 eyes in each group,Chinese medicine fumigation group,intense pulsed light group,and Chinese medicine fumigation combined with intense pulsed light group.The traditional Chinese medicine fumigation group was given soothing wind and clearing heat Chinese medicine fumigation,once every 4 days,for a total of 8 times;the intense pulsed light group was given intense pulsed light treatment,once every 14 days,a total of 3 times;the traditional Chinese medicine fumigation combined with the intense pulsed light group was given traditional Chinese medicine Fumigation treatment(once every4 days,a total of 8 times);at the same time,intense pulsed light treatment(once every 14 days,a total of 3 times).Before and after treatment,the subjective ocular symptom score,eyelid margin change score,meibomian gland secretion discharge ability score,meibomian gland secretion trait score,meibomian gland absence score,corneal fluorescein staining(FL)score and noninvasive breakup time(NIBUT).Result:(1)Comparison of subjective eye symptom score,eyelid margin change score,and meibomian gland secretion trait score: after treatment,the subjective eye symptom score,eyelid margin change score,and meibomian gland secretion trait score of the three groups of patients were higher than those before treatment.The scores of the traditional Chinese medicine fumigation combined with intense pulsed light group were lower than those of the intense pulsed light group and the traditional Chinese medicine fumigation group,and the scores of the intense pulsed light group were lower than those of the traditional Chinese medicine fumigation group,and the differences were statistically significant(P<0.05).(2)NIBUT comparison: after treatment,the NIBUT of the three groups was longer than that before treatment,and the NIBUT of the traditional Chinese medicine fumigation combined with intense pulsed light group(12.19±1.85s)was higher than that of the intense pulsed light group(11.31±1.47s)and the traditional Chinese medicine fumigation group(10.17±1.65s),and the NIBUT in the intense pulsed light group was higher than that in the traditional Chinese medicine fumigation group,with a statistically significant difference(P<0.05).(3)Comparison of meibomian gland secretion discharge ability score and FL score: after treatment,the meibomian gland secretion discharge ability score and FL score of the three groups were lower than those before treatment,and the score of the traditional Chinese medicine fumigation combined with intense pulsed light group was lower than that of the traditional Chinese medicine fumigation group and the intense pulsed light group,the difference was statistically significant(P<0.05).There was no significant difference in the meibomian gland secretion discharge ability score and FL score between the traditional Chinese medicine fumigation group and the intense pulsed light group(all P>0.05).(4)Comparison of Meibomian Gland Missing Scores: There was no significant difference in the Meibomian gland missing scores between the three groups after treatment(all P>0.05).There was no significant difference in the absence of meibomian glands among the three groups after treatment(P>0.05).Conclusion:Traditional Chinese medicine fumigation combined with intense pulsed light can improve the tear film breakup time in patients with dry eye due to meibomian gland dysfunction,improve ocular symptoms,eyelid margin and meibomian gland openings,and meibomian gland secretion properties,promote meibum discharge,and reduce corneal Fluorescein sodium staining.Traditional Chinese medicine fumigation combined with intense pulsed light therapy is better than pure intense pulsed light therapy and traditional Chinese medicine fumigation alone.
