Effect Of Early Plantar Fascia Stress Training On Foot Rehabilitation After Calcaneus Fracture Surgery

Background:Calcaneal fracture is a common clinical fracture,mostly caused by vertical violence,often involving the articular surface,and can cause soft tissue injury around the calcaneal bone,especially the plantar bone and fascia injury.No matter surgical treatment or rehabilitation exercise is focused on plantar related treatment,which may affect the overall treatment and rehabilitation effect.Objective:Based on the theory of "equal emphasis on muscles and bones",this study aims to achieve better therapeutic effect by early intervention of plantar fascia stress training after surgery,combined with muscle strength of ankle and lower limb and ankle joint exercise.Methods:A total of 70 patients with calcaneal fracture Sanders II and III who were hospitalized for surgical treatment from January 2021 to September 2021 were selected,and their syndrome was qi stagnation and blood stasis type.Two groups were randomly divided into two groups,35 cases in control group and 35 cases in experimental group.In the control group,early foot and ankle training was used for postoperative rehabilitation.The commonly used rehabilitation methods were mainly tendon and muscle stretching,active and passive joint functional exercise.In the experimental group,on the basis of early foot and ankle training,the early plantar fascia stress training was carried out with plantar stress training rehabilitation equipment.The postoperative pain levels of patients were compared and recorded on the 1st and 3rd postoperative days,and the postoperative detumescence days of patients were counted during hospitalization.After that,clinical healing time of postoperative calcaneal fracture,AOFAS score 3 months after surgery and 6 months after surgery,percentage of affected foot pressure in total pressure of both feet 6 months after surgery,proportion of healthy side support phase in total gait cycle of healthy side 6 months after surgery were recorded and analyzed.Results:1.General conditions: the general conditions(gender,age,fracture type)of the two groups of subjects were compared,and the results were not statistically significant(P > 0.05),indicating that there was no significant difference in the general conditions of the two groups of subjects,which could be compared.2.Efficacy results:(1)Postoperative pain grade evaluation The pain scores of the two groups were observed on the 1st and 3rd day after surgery,and the intra-group comparison was conducted.The pain scores on the 1st day after surgery were not statistically significant(P > 0.05),while the pain scores on the 3rd day after surgery were statistically significant(P < 0.05).(2)Time of postoperative skin striations the time of postoperative skin striations was observed between the two groups,and the difference was statistically significant(P < 0.05).(3)Clinical fracture healing time The clinical fracture healing time of patients in the two groups was evaluated and recorded through regular postoperative review,with no statistical significance(P > 0.05).(4)AOFAS scores 3 months and 6 months after surgery Were used to evaluate the function of patients 3 months and 6 months after surgery by AOFAS score scale,and the differences were statistically significant(P < 0.05).(5)Percentage of the affected lateral foot pressure in the total pressure of both feet six months after surgery.The percentage of the affected lateral foot pressure in the total pressure of both feet was obtained through the analysis of the plantar stress of the patients,and the difference was statistically significant(P < 0.05).(6)Percentage of healthy side supporting phase in the total gait cycle of the healthy side6 months after surgery.Through gait analysis of the patients,the proportion of healthy side supporting phase in the whole walking cycle of the healthy side was counted.The difference was statistically significant(P < 0.05).Conclusion:In this study,under the guidance of the theory of "equal emphasis on muscles and bones" and the integration of traditional Chinese medicine Massage Therapy the early intervention of plantar fascia stress training combined with foot and ankle functional training was found to have a positive effect on the improvement of early postoperative pain and swelling and functional recovery.
