| Purpose:The purpose of this subject is to study the clinical effect of acupoint catgut embedding combined with chronic urticaria decoction on chronic urticaria(Qi deficiency and blood stasis type)and its influence on the change of D-dimer level,so as to demonstrate the curative effect advantage of traditional Chinese medicine from clinical research.To further explore the correlation between coagulation/fibrinolysis mechanism and the disease,so as to provide new methods and expand new ideas for the treatment of chronic urticaria.Methods:90 patients with chronic urticaria of qi deficiency and blood stasis type are planned to be included and randomly divided into three groups.Acupoint catgut embedding combined with chronic urticaria decoction is the treatment group,chronic urticaria decoction alone is the control group I,and Western medicine group adopts oral ebastine tablets as the control group II.The final number of cases is 88,including 30 cases in the treatment group,29 cases in the control group I and 29 cases in the control group II.Ask the patient to draw venous blood on an empty stomach for plasma D-dimer test before the first treatment,and those who are positive will be retested after the end of the treatment cycle.The adverse reactions of patients during treatment were monitored and treated actively.The recurrence of patients in each group 8 weeks after treatment was counted,and the above statistical data were statistically analyzed.Results:1.Baseline comparison:excluding 2 cases,88 cases were finally included.There was no significant difference in gender,age,course of disease,symptom score of chronic urticaria before treatment,TCM syndrome score and quality of life score of chronic urticaria among the three groups(P>0.05).2.Symptom score of patients with chronic urticaria:comparison within the group:the three groups were significantly improved in pruritus,skin lesion distribution,wind mass size,wind mass duration,attack frequency and total score(P<0.05);Comparison between groups:after treatment,the distribution of skin lesions,the size of wind mass and the frequency of attack in the treatment group were significantly lower than those in the control group Ⅰ(P<0.05)and the control group Ⅱ(P<0.05);The scores of pruritus and duration of wind mass in the treatment group were significantly lower than those in the control group Ⅱ(P<0.05),but there was no significant difference compared with the control group Ⅰ(P>0.05).3.TCM syndrome score:intra group comparison: the three groups have significantly improved in TCM syndrome,and the difference is statistically significant(P<0.05);Comparison between groups:the TCM syndrome score of the treatment group was significantly lower than that of the control group Ⅰ(P<0.05)and the control group Ⅱ(P<0.05).4.Quality of life score of chronic urticaria:intra group comparison:the three groups have significantly improved in TCM syndrome,and the difference is statistically significant(P<0.05);Comparison between groups:the comparison results between the treatment group and the control group I and the control group II were all P<0.05,and the difference was statistically significant.5.Clinical efficacy:Treatment group:a total of 30 patients,of which4 cases were cured,20 cases were markedly effective,4 cases were effective and 2 cases were ineffective.The total effective rate was 93.33% and the recovery rate was 80%.Control group I: a total of 29 patients,including2 cases cured,8 cases markedly effective,14 cases effective and 5 cases ineffective.The total effective rate was 80% and the recovery rate was33.33%.Control group II:a total of 29 patients,including 1 case cured,6cases markedly effective,15 cases effective and 7 cases ineffective.The total effective rate was 73.33%,and the recovery rate was 23.33%.There was no significant difference in the total effective rate between the treatment group and the control group I and the control group II(P>0.05).There were significant differences between the treatment group and the control group ⅰ and the control group ⅱ(P<0.05);There was no significant difference between control group I and control group II(P>0.05).6.Before treatment,the correlation between the plasma D-dimer level and the total score of chronic urticaria symptoms in the three groups was analyzed.The differences were statistically significant(P <0.05),showing a positive linear relationship.There was no significant difference in the positive rate of plasma D-dimer among the three groups before treatment(P > 0.05);After treatment,there was no significant difference between the treatment group and the control group I(P >0.05);There was significant difference between treatment group and control group ⅱ(P<0.05);There was no significant difference between control group I and control group II(P>0.05).7.Recurrence rate:the recurrence rate of the three groups was compared between the treatment group and the control group I and the control group II(P<0.05).Conclusion:1.Acupoint catgut embedding combined with chronic urticaria decoction has unique advantages in improving the clinical symptoms of chronic urticaria of qi deficiency and blood stasis type,and can significantly improve the quality of life of patients.There is no significant difference between manurticaria formula alone and Western medicine group.2.In this study,D-dimer in plasma is closely related to the severity of chronic urticaria.At the same time,acupoint catgut embedding combined with manurticaria formula has more advantages in improving blood hypercoagulability.3.The pathogenesis of chronic urticaria is complex.Acupoint catgut embedding combined with manurticaria formula has obvious advantages in the treatment of chronic urticaria of qi deficiency and blood stasis type,which enriches the clinical treatment methods and has broad prospects in clinical development. |
