| Objective:Two common Parks types(intersphincteric type and transsphincteric type)anal fistula were retrospectively measured by MRI image data,and the thickness of internal and external anal sphincter was collected.The correlation between TCM syndrome types and TCM syndrome types provides an objective reference for the anatomical basis of common Parks type anal fistulas and TCM treatment.Methods:This study retrospectively analyzed 86 male patients with anal fistula confirmed by surgery in the Department of Anus and Proctology of Jingdezhen Traditional Chinese Medicine Hospital from January 2018 to July 2020,including 60 cases of intersphincteric type and 26 cases of transsphincteric type.All patients completed perianal MRI examination and collection of TCM syndrome differentiation within 2days before operation.By measuring the thickness of the internal and external anal sphincter in patients with intersphincteric and transsphincteric anal fistulas on perianal MRI images,the differences in anal sphincter thickness in patients with two common Parks type anal fistulas were explored.The measurement method of anal sphincter thickness is as follows: Select the T1 WI axis The internal and external sphincter thickness were measured at the mid-section of the axial anal canal and the largest slice showing the anal sphincter.Take 3or 9 points(first take the measurement data on the normal side,if the lesion is present at the two points,select adjacent points instead).According to TCM syndrome differentiation,patients were divided into damp-heat betting type and positive deficiency and evil love type.The differences in the distribution of TCM syndrome types in patients with two Parks type anal fistulas were compared,and the correlation between common Parks type anal fistulas and TCM syndrome types was analyzed.Results:The measurement and analysis of the thickness of the internal and external anal sphincter in patients with sphincteric and transsphincteric anal fistulas was carried out.Statistical analysis showed that the difference in the thickness of the external sphincter between the two was statistically significant(P<0.05).mm,after statistical analysis,there was no significant difference in the thickness of the internal sphincter between the two Parks type anal fistula patients(P>0.05).After TCM syndrome diagnosis,there were 26 cases of transsphincteric anal fistula,including 11 cases of damp-heat injection type(42.3%)and 15 cases of positive deficiency and evil love syndrome(57.7%);44 cases(73.3%)with injection type and 16 cases(26.7%)with positive deficiency and evil love syndrome.Using chi-square test,it was found that there were significant differences in TCM syndrome types between the two groups of patients with different Parks type anal fistulas(P<0.05).),in the intersphincteric anal fistula,the proportion of damp-heat injection syndrome is more,and the positive deficiency and evil love syndrome are mostly distributed in the transsphincteric anal fistula.General clinical data such as age,disease duration,hyperlipidemia,diabetes mellitus,smoking history,and alcohol consumption history were not significantly different between the two groups.Conclusion:Different thicknesses of external anal sphincter may have an impact on the process of forming different Parks type anal fistulas.Common Parks type anal fistulas are correlated with TCM syndrome types;MRI can provide reference for the anatomical basis of the two Parks anal fistula types and TCM adjuvant treatment in accordance with. |
