Objective:In cataract surgery,the lens capsule is an important place for many surgical steps,and postoperative it is a permanent place for IOL fixation.However,the diameter of the capsule can not be measured directly clinically,which can only be inferred from other measurements.Corneal diameter is most commonly used clinically.Therefore,this paper compares the incidence of capsule folds of different corneal diameters and their impact on visual quality,so as to guide the choice of IOL after cataract extraction and reduce postoperative complications.Methods:This study was conducted using a prospective approach and was divided into two sections.The first part selected 39 eyes of 41 age-related cataract patients who underwent standard phacoemulsification combined with IOL implantation in our hospital between June 2021 and December 2021,Patients were divided into two groups according to implanted L diameter,In Group A,in 18 patients with 18 eyes,With an implanted optical diameter of 6.0mm,Four-antennal hydrophilic acrylate IOL with a total length of 10.7mm(Ph.D.AO,USA),In group B,in 21 patients with 23 eyes,With an implanted optical diameter of 6.0mm,Three-piece hydrophobic acrylate IOL with a total length of 12.5mm(HOYA P250 D,Japan).All patients completed a series of routine preoperative cataract examinations for the age,sex,ocular axis,corneal diameter,IOL diameter,preoperative naked eye vision and best corrected visual acuity of the patients included in the study.Patients with intraoperative posterior capsule striations were recorded.The incidence of the posterior capsule fold was compared between the two groups.The process of changes in the recorded posterior capsule fold was observed at 1 day,1 week,1 month,and 3 months under a slit-lamp microscope.The second part divided all patients into fold group and the fold-free group from the original case,and compared the incidence of corneal diameter,best corrected visual acuity,1 day,1 week,1 month and 3 months.Results:The total incidence of capsule folds after both IOL bag implantation was 27%,11% and 39%,respectively,with p=0.044 <0.05,a significant difference.The p-value was <0.05.The comparison of the corneal diameter between the wrinkled and non-wrinkled groups was p=0.003 <0.05.The best corrected visual acuity p-value was> 0.05.The incidence of postoperative visual interference in the two groups was55% in the fold group and 7% in the non-fold group,with p=0.003 <0.05 obtained by the chi-square test.Conclusion:1.When the corneal diameter is in the range from 11.0mm~12.0mm,the incidence of capsule ruffling after implantation of IOL with the total length of12.5mm is higher than that of IOL with the total length of 10.7mm.The IOL length and the corneal diameter are the important influencing factors of the posterior capsule folds,which are more likely to occur in patients with a shorter corneal diameter.2.posterior capsule wrinkle has no impact on visual vision after cataract surgery,but it is easy to cause visual interference and glare,and reduce the postoperative visual quality.
