Correlation Of Triglyceride-glucose Index With White Matter Hyperintensity In Non-diabetic Patients

Objective : The purpose of this study is to analyze the association between triglyceride-glucose(TyG)index and the white matter hyperintensity(WMH)in nondiabetic patients,and to investigate whether TyG index is a risk factor for WMH.Methods:The present study is a single-center retrospective case-control study.We identified 164 non-diabetic hospitalized patients in the neurology department in Ganzhou Municipal Hospital from March 2021 to September 2021 according to our inclusion and exclusion criteria.By head MRI evaluation imaging,all patients were divided into case group with WMH and control group without WMH(WMH was classified as light,moderate and severe based on the Fezakas grade).We compared the clinical characteristics including age,sex,height,weight,comorbidities,blood pressure,triglycerides,HDL-C,HCY,uric acid,creatinine,LDL-C,cholesterol,and TyG index between two groups.We also focused on investigating the correlation of TyG index with high signal severity in brain white matter.Results:(1)Comparison of risk factors: compared with the non-WMH group,there were significant statistical differences between two groups in age(54.5±11.1vs 67.3 ± 10.9,P <0.001),history of hypertension(22.4%vs 68.7%,P <0.001),triglycerides /mmol/L(1.22 vs 1.61,P <0.001),high density lipoprotein / mmol/L(1.39 vs 1.21,P=0.008),homocysteine mmol/L(10.7vs 11.8,P=0.013),TyG index(8.46 vs 8.83,P<0.001).Multivariate logistic regression analysis showed that age(OR=1.131,95%CI:1.078-1.186)and hypertension(OR=5.69,95%CI:2.143-15.109),(OR=7.868,95%CI:1.794-34.517)were independent risk factors of WMH.(2)There was no significant difference between TyG index and WMH severity grade(P=0.553).(3)Spearman correlation analysis between TyG index and other risk factors showed that TyG index level showed a positive correlation with uric acid,blood sugar,cholesterol,triglycerides,body weight,and Hb A 1 c(P <0.05),and a negative correlation with HDL(P <0.05).Conclusion:(1)Age,hypertension,triglycerides,high-density lipoprotein,homocysteine,TyG index corassociated with WMH.Age,hypertension,and high TyG index were independent risk factors for WMH.(2)There was no correlation between TyG index and WMH.(3)The TyG index may be correlated to risk factors such as hypertension,abnormal lipid metabolism and hyperuricemia.
