Research On The Application Of Self-developed Hearing Self-test Applet Based On Wechat In Nursing Assessment Of Cisplatin Ototoxicity

Objective:To develop a hearing self-test applet based on WeChat platform and evaluate the feasibility of the applet as an ototoxicity nursing evaluation tool.Methods:According to the needs of clinical nursing assessment of ototoxicity,a Wechat based hearing self-test applet was designed and developed to compare the hearing threshold test results of the hearing self-test applet and the pure tone audiometer in 15 normal hearing people(30 ears)and 47 outpatient patients in otology,and to evaluate the accuracy of hearing test and hearing screening using the applet;Compare the time required for hearing test between the self-test program and the pure tone audiometer.Sixty two cisplatin chemotherapy patients were recruited to assess the ototoxicity of cisplatin using the gynecological tumor patients’ neurotoxicity assessment scale(FACT/GOG-NTx)and the hearing self-test applet as nursing assessment tools.The correlation between the assessment results of the above two assessment methods and the results of pure tone audiometer was analyzed,and the system availability evaluation of the applet by patients and nurses was collected.Results:There was no significant difference between the test results of WeChat hearing self-test applet and the audiometry in normal hearing people(P>0.05).The results of hearing impairment screening in patients of otology clinic showed that the sensitivity was 93%and the specificity was 86.2%.The time of hearing test with self-test program(5.67 ± 1.1 min)was significantly less than that of pure tone audiometer(13.6 ± 2.44 min),the difference was statistically significant(P<0.05).The sensitivity and specificity of the hearing self-test applet for the screening of cisplatin ototoxicity were 91.3%and 87%.There was a good consistency between the results of hearing self-test applet and that of pure tone audiometry(Kappa=0.76).As a nursing observation tool,the positive evaluation report of the hearing self-test applet was significantly correlated with the audiometry examination results(r=0.65,P=0.001);In contrast,there was no significant correlation between the evaluation results of the FACT/GOG-NTx scale and the results of audiometry examination(r=0.03,P=0.74).The SUS score of the hearing self-test applet was 76.3 ± 1.65 in patients and 80.33 ± 1.43 in nursing staff.The doctors’satisfaction for the hearing self-test applet was 4.62 ± 0.69,and 3.53 ± 0.8 for the FACT/GOG-NTx scale,the difference was statistically significant(P=0.021).Conclusion:The hearing self-test applet based on WeChat platform developed by us has significantly higher screening efficacy for cisplatin ototoxicity than the FACT/GOG-NTx scale.The satisfaction of medical staff and patients is high.It is expected to be an effective auxiliary tool for clinical nursing evaluation of cisplatin ototoxicity.
