Objective:By detecting the expression of vitamin D receptor(VDR)in tissues and the expression of vitamin D in serum between erosive and non-erosive oral lichen planus(OLP)group,to study the role of vitamin D and its receptor in the development of OLP and the correlation between serum vitamin D level and VDR in OLP tissues.Methods:According to the inclusion and exclusion criteria,33 OLP patients in Shanxi Provincial People’s Hospital were selected,including 14 erosive OLP patients(erosive OLP group),13 non-erosive OLP patients(non-erosive OLP group),6 OLP patients who had erosive lesions on one buccal mucosa and non-erosive lesions on the other side(combined erosive and non-erosive OLP group),14 healthy volunteers were selected as the control group.The pathological tissues of OLP patients and normal oral mucosa of normal controls were collected,and all tissue samples were confirmed by pathology.Immunohistochemical technique was used to detect the expression of VDR in each group.The expression of VDR m RNA in tissues was detected by real-time fluorescent quantitative PCR(q PCR).Collect blood samples of patients in erosive OLP group and non-erosive OLP group,serum 25-hydroxy-vitamin D[25-hydroxy-vitamin D,25(OH)D]levels in patients with erosive OLP group and non-erosive OLP group were measured by electrochemiluminescence,and the correlation between serum vitamin D levels and VDR protein and gene expression was observed.Results:The expression of VDR protein and VDR m RNA in erosive and non-erosive OLP group are lower than those in normal control group,and the erosive OLP group is lower than that in non-erosive OLP group,but only the difference between normal control group and erosive OLP group is statistically significant(P<0.05),while the other differences were not statistically significant(P>0.05).In OLP patients with erosive lesions and non-erosive lesions,VDR m RNA in erosive lesions was lower than that in non-erosive lesions,but the difference was not statistically significant(P>0.05).Serum25(OH)D levels of all patients in erosive and non-erosive OLP groups were lower than normal,but there was no significant correlation between serum 25(OH)D level and VDR protein and VDR m RNA expression in OLP lesions(r1=0.125,p1>0.05;r2=-0.084,p2>0.05)。Conclusion:The expression of VDR was low in erosive OLP lesions,suggesting that the lack of VDR might be involved in the formation of erosive OLP.There was no significant correlation between serum 25(OH)D and VDR expression in OLP lesions,suggesting that oral vitamin D or VDR agonist may not be effective in the treatment of OLP,and local administration may be more appropriate.In the next step,we need multi-center and large sample research to verify it,and further basic experiments are needed to explore the mechanism of vitamin D and VDR in the occurrence and development of OLP.
