| Objective: By constructing an environmental disinfection and sterilization program,it is applied to the management of nosocomial infection in burn wards,the effect of air disinfection and purification in burn wards is observed,and the influence on infection of burn patients is explored,so as to provide support basis for guiding and improving the standard management of infection control in burn ward.Methods: This research is a kind of experimental research,which is divided into experiment one,experiment two and experiment three parts.(1)Experiment 1 Selected several unmanned general wards of burn patients in a tertiary hospital in Wuxi City after discharge,and collected 60 sets of data to determine the air purification effect of the environmental disinfection and sterilization scheme in unmanned general wards.They were divided into experimental group and control group according to whether the environmental disinfection and sterilization plan was applied.The air in the ward was disinfected for half an hour every two days,and the bacterial count and air particle value were monitored before and after disinfection to compare the air purification effect.In order to explore the dynamic disinfection effect of the application of the environmental disinfection and sterilization scheme in the burn ward with patients,two general wards with patients were randomly selected in the burn department,and the rest were the same as the research design and grouping of unmanned wards.(2)Experiment 2 Selected several Burn Intensive Care Units(BICUs)of unmanned burn patients after discharge from a tertiary hospital in Wuxi City,and collected a total of 60 sets of data to determine the application environment in unmanned BICUs.Air purification effect of disinfection and sterilization programs.Afterwards,two BICUs with patients were randomly selected in the burn department,and the rest of the groupings and research design were the same as those of Experiment 1.(3)In Experiment 3,a total of 120 burn patients who met the admission and discharge standards in a tertiary hospital in Wuxi City were selected to explore the effect of environmental disinfection and sterilization program on the infection of burn patients(60 cases)and BICU patients(60 cases),as long-term disinfection.The experimental group and the control group were grouped in a similar manner to that of the first experiment.Air disinfection was performed 24 hours a day.The infection rate and related infection indicators of burn patients in the experimental group and the control group in the general ward and the BICU medium and long-term disinfection group were compared respectively.And the evaluation of the quality of hospital sense management in the ward,the individual comfort of patients,and the self-satisfaction of nursing staff were compared.index.To compare and judge the disinfection effect of the environmental disinfection and sterilization program in different wards of the burn department.Results:(1)In the air purification group,there were statistically significant differences in the number of air particles and colonies in each ward between the experimental group and the control group(P<0.05).After disinfection,the number of air particles and bacterial colonies in the ward of the experimental group were significantly reduced(P<0.05),while the air data of the control group had no obvious downward trend(P>0.05).After 10 minutes of disinfection,the air particulate matter in the experimental group had the largest decrease,and then the decreasing trend gradually became gentle;the decreasing trend of the particulate matter in the control ward was not obvious.The sterilization rate of the experimental group ward was much higher than that of the control group(the sterilization rate of the general ward was 89.4%>-16.8%,and the sterilization rate of the BICU was 90.7%>-1%).(2)Comparing the information of patients in the general ward and BICU respectively,it was found that the baselines of the patients in the two groups were the same.There was no statistical difference in the distribution of nutritional status and other constituent ratios(all P>0.05).The average hospitalization days of the patients in the control group and the experimental group in the general ward were(20.56±9.96)days and(17.29±8.06)days,respectively.The average length of stay in the BICU control group and experimental group were(29.16±17.76)days and(20.93±9.86)days,respectively.However,the positive rate of bacterial culture of wounds in the long-term disinfection control group was much higher than that in the experimental group(P<0.05).The infection rate of burn patients in the experimental group(60.00% in the general ward and 53.33% in the BICU)was significantly lower than that in the control group(93.33%in both the general ward and the BICU).(3)Although the overall data of each group approached the normal range with the increase of hospitalization time,the laboratory index values of the patients in the experimental group had a greater decrease and a more obvious trend than that of the control group.(4)The overall pass rate of the quality assessment of ward hospital infection management in the experimental group(91.33%)was significantly higher than that in the control group(78.00%).Before and after disinfection in the control group,before and after disinfection in the experimental group and after disinfection in the experimental group,there were significant differences in the overall comfort level and the comfort level of the environmental dimension(P<0.05).The satisfaction rate of each dimension in the self-job satisfaction survey of nursing staff in the experimental group was higher than that in the control group(P<0.05).Conclusion: In this study,an environmental disinfection and sterilization program was constructed and applied to the nursing management of nosocomial infection in burn wards.It was found that environmental disinfection can effectively improve the air quality in the ward.Compared with the control group,the infection rate and related infection indicators of burn patients who applied the environmental disinfection and sterilization program were improved to a greater extent,the qualified rate of the quality evaluation of hospital sense management in the ward was higher,as well as the personal comfort of patients and the self-satisfaction of nursing staff both increased significantly.To sum up,in order to reduce the infection rate of burn patients,shorten the hospitalization time,improve the efficiency of infection control,and save nursing manpower and time cost,this study adopted an environmental disinfection and sterilization scheme to ensure the effective management of in-hospital infection care in burn wards,and provided a reference for the formation of a complete nosocomial infection control plan. |
