Objective:The purpose of this study is to use the method of questionnaire survey to understand the cognition,contract and demand of family doctor services among urban residents in Jinan,and on this basis,analyze the influencing factors of the demand for contract services of family doctors,in order to carry out family doctor contract service better,so as to improve the phenomenon of "signing for signing" and "signing without making an agreement",and provide a reference for the realization of "signing and making an agreement" for family doctor signing service.Methods:A multi-stage random sampling method was used to select 1,000 eligible residents from 5 urban areas of Jinan City(Shizhong District,Tianqiao District,Licheng District,Zhangqiu District,and Laiwu District)for investigation,and a total of 948 valid questionnaires were collected.The data were sorted and summarized using Excel 2017 software,and SPSS 24.0 statistical software was used for statistical analysis.Frequency and percentage were used to describe the demographic characteristics of urban residents in Jinan and the status quo of cognition,contract signing and demand for family doctor services;principal component analysis was used to conduct exploratory factor analysis on the scores of 12 service demand items;t-test,variance to analyze and compare the differences of the four service demand common factors and comprehensive demands of urban residents with different characteristics,and use the multiple linear regression method to explore the influencing factors of urban residents’ different demand for family doctor contract services.Results:Among the 948 surveyed urban residents,83(8.76%)knew very well about the contracted services of family doctors,108(11.39%)knew well,and 276(29.11%)knew little about it.There are 385 people(40.61%)who do not understand,and 96people(10.13%)who do not know at all;428 of the surveyed residents People(45.15%)have signed up with family doctors,of which 332(77.57%)residents believe that family doctors are helpful to them during the prevention and control of the new crown pneumonia epidemic,246 people(28.90%)have not signed up,and274 people(28.90%)was not clear;in the survey on willingness to sign up,715 people indicated that they were willing to sign up for family doctor services,accounting for 75.42% of the surveyed population.In the survey of the demand for family doctors contract services,180 people(18.99%)said they needed it very much,199 people(20.99%)needed it more,369people(38.92%)needed it in general,and 136 people needed it less.people(14.35%),64 people(6.75%)do not need it at all.The scores of 12 demand service items are between 2.59~3.54,and the top three projects with demand score are: key population health management service(3.54 ± 0.987),special needs door-to-door service(3.37 ± 0.997),infectious disease prevention and control guidance services(3.33±1.238).After factor analysis,the 12 demand service items were synthesized into 4 common factors,namely: personalized service demand,basic medical service demand,public health service demand,and health management service demand.The influencing factors were marital status,education level,occupation,Household per capita monthly income,preferred medical institution(P<0.05),gender,marital status,education level,occupation,household per capita monthly income,local household registration,self-assessed health status,preferred medical institution(P<0.05),gender,age,marital status,education level,occupation,per capita monthly household income,self-assessed health status,chronic disease,long-term medication,physical discomfort within two weeks(P<0.05),age,marital status,occupation,The per capita monthly income of the family,whether it is a local household registration,self-assessed health status,whether suffering from chronic diseases,whether taking medicine for a long time,whether you are unwell within two weeks,and the first medical institution(P<0.05).Conclusions:The overall cognition level of urban residents in Jinan is higher than that of rural residents,but it still needs to be improved compared with areas with high economic development level.The reports on family doctor services during the COVID-19 epidemic have played a positive role in residents’ cognition.The overall contracting rate of family doctor services is not high,but it is worth mentioning that most of the contracting residents said they had received effective services provided by family doctors during the new crown pneumonia epidemic,and the overall willingness of residents to contract is also high.Residents’ demand for contracted family doctor services is mainly focused on personalized services.Compared with the previous project,the demand for infectious disease prevention guidance services has been greatly improved.In addition,the demand for physical examination services has always been at the forefront.Urban residents with different characteristics have different needs for family doctors’ personalized services,basic medical services,public health services,and health management services.The satisfaction of the residents is to encourage residents to realize independent signing and signing contracts. |
