| Objective:According to observe the intervention effect of auricular acupressure in patients with uremic restless legs syndrome,comprehensively evaluate the impact of this therapy in improving TCM symptoms,severity of restless legs syndrome,quality of life,laboratory examination and safety.So as to provide a safety and effective TCM nursing method to improve the clinical symptoms of patients with uremic restless legs syndrome.Methods:The convenience sampling method was used to select 66 dialysis patients with uremic restless legs syndrome and TCM dialectical typing of spleen-kidney qi deficiency with blood stasis from September 2020 to April2021 in the hemodialysis department of a tertiary care TCM hospital in Harbin,who met the inclusion and exclusion criteria.They were randomly divided into test group and control group,with 33 cases in each group.The control group was treated with conventional dialysis treatment and primary care;Based on the control group,the test group received auricular point seed pressing therapy after the first dialysis every week.Selected points are Spleen,Liver,Kidney,Shen Men,Sympathetic,Adrenal and Small Occipital Nerve Points,pressed 4 times a day in the moring,middle,evening and before bedtime.Each time each point continues to be pressed for 1~3 minutes,seven days was a course of treatment,and there are 6 courses in total.Before and after intervention,the intervention effect was comprehensively evaluated by the TCM syndrome scale,International Restless Legs Syndrome scale,SF-36Quality of Life scale and Related laboratory tests.Results:1.Before the intervention,the general information,TCM syndrome scale,IRLS scale,SF-36 Quality of Life scale and Related laboratory tests were compared between the two groups,and the differences were not statistically significant(P>0.05),indicating that the two groups were consistent at baseline level and could be compared.2.TCM syndrome score:(1)within-group comparison before and after the intervention,the total score and single score of TCM syndromes in both groups were statistically significant(P<0.05),indicating that the TCM syndromes in both groups were improved after the intervention;(2)the difference between two groups after the intervention was statistically significant(P<0.05),indicating that the test group was superior to the control group in terms of improving the TCM syndromes.3.Efficacy of TCM syndrome inte:the total effective rate of the test group was 93.5%,and that of the control group was 65.6%.The difference between the two groups was statistically significant(Z=-3.436,P<0.05),indicating that compared with the control group,the test group can better improve the effect of the TCM syndromes.4.IRLS scale scores:(1)within-group comparison before and after the intervention,the scores in both groups were decreased,and the difference was statistically significant(P<0.05),showing that the severity of restless leg syndrome was reduced in both groups;(2)the test group showed better improvement in the severity of restless leg syndrome than the control group after the intervention,and the difference was statistically significant(P<0.05).5.SF-36 Quality of Life Scale scores:(1)within-group comparisons before and after the intervention,the scores of the test group in Physical Functioning,Role-Physical,Bodily Pain,Vitality,Social Functioning,Mental Health and total scores were statistically significant(P<0.05);and the control group have significant differences in Physical Functioning,Bodily Pain,Role-Emotional and total scores(P<0.05);(2)the comparison between two groups after intervention in Physical Functioning,Role-Physical,Bodily Pain,Vitality,Social Functioning,Mental Health and total scores were statistically significant(P<0.05),all scores in the test group were higher than those in the control group;While there was no significant difference in General health and Role-Emotional(P>0.05).6.Laboratory examination:(1)within-group comparisons before and after the intervention,laboratory indicators such as parathyroid hormone,blood phosphorus,blood calcium andβ2-microglobulin were improved in both groups,and the difference was statistically significant(P<0.05);(2)the test group improved better than the control group after the intervention,and the difference between the two groups was statistically significant(P<0.05).7.Safety evaluation:The safety level of this study was first grade,and there were no related adverse reactions and abnormal changes in the two groups.Conclusion:1.Auricular acupressure can effectively reduce the severity of uremic restless legs syndrome in dialysis patients,and promote the outcome of TCM syndrome and improve clinical symptoms.2.Auricular acupressure can promote the metabolism of toxins associated with uremia,and improve the quality of life of patients with uremic restless legs syndrome.3.The auricular acupressure has the characteristics of high safety,simple operation,suitable price and high acceptance of dialysis patients,so it is worth popularizing in clinic. |
