Study On The Law Of Medicine Use And Experience Summary Of Professor Ma Jian In Treating Hyperthyroidism

Objective:Based on the Traditional Chinese Medicine Inheritance Computing System(TCMICS)(V3.0),this study studied the medication rule of Professor Ma Jian in the treatment of hyperthyroidism,and summarized his clinical experience and academic thoughts in the treatment of hyperthyroidism.It provides new ideas and opportunities for further developing the treatment and application of traditional Chinese medicine in hyperthyroidism.Methods:A total of 185 outpatient cases of patient,which met the inclusion and exclusion criteria and were diagnosed with hyperthyroidism from January 2017 to January 2020 by Professor Ma Jian who worked at the Department of Endocrinology,The First Affiliated Hospital of Heilongjiang University of Traditional Chinese Medicine were recorded.After standardizing the recorded data,import it into the Traditional Chinese Medicine Inheritance Computing System(TCMICS)(V3.0)through the Excel template.Then the basic information of patients,symptoms,tongue and pulse information,drug frequency information,drug taste meridian and efficacy information and prescription rules were analyzed,so as to summarize the medication rule,clinical experience and academic thought of Professor Ma Jian in the treatment of hyperthyroidism.Results:(1)In 185 cases of hyperthyroidism,female patients were more than male patients,with a ratio of 4.97:1.The age of onset was mainly between 20 and 49 years old,accounting for 76.21%of the total.(2)In 185 cases of hyperthyroidism,134 symptoms were found,and palpitations were the most frequent.(3)In 185 cases of hyperthyroidism,a total of ten tongue manifestations were obtained,the main tongue manifestations were red tongue,white tongue coating and tooth marks;a total of 19 pulse patterns were obtained,the string,slip and fast pulse is the main pulse.(4)Among 185 prescriptions,127 drugs were used,and poria cocos was the most frequently used.(5)In 185 cases of prescription drugs,the drug property was dominated by cold drugs,warm drugs and neutral drugs;the medicine flavor was dominated by bitter drugs,sweet drugs and pungent drugs;the meridians of lung meridians,spleen meridians,liver meridians,heart meridians,kidney meridians,stomach meridians and gallbladder meridians were more than 500,they accounted for 91.37%of all frequencies.(6)In 185 cases of prescription drugs,the drug efficacy is mainly tonifying deficiency and clearing away heat.(7)Based on association rules,the core drugs were as follows:Poria Cocos,Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Peony Bark,Radix Paeoniae Alba and Gardenia Jasminoides.(8)Six core drug combinations were obtained based on k-means algorithm clustering,which were as follows:Core prescription 1:Poria Cocos,Radix Bupleuri,Radix Paeoniae Alba,Pericarpium Citri Reticulatae,Rhizoma Atractylodis Macrocephalae,Bulbus Fritillariae Thunbergii,Radix Platycodi,and Radix Angelicae Sinensis;Core prescription 2:Poria Cocos,Cortex Moutan,Gardenia jasminoides,Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Radix Paeoniae Alba,Scutellaria Baicalensis,Radix Angelicae Sinensis;Core prescription 3:Poria Cocos,Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Cortex Moutan,Licorice,Gardenia Jasminoides,Radix Paeoniae Alba,Pericarpium Citri Reticulatae;Core prescription 4:Poria Cocos,Cortex Moutan,Gardenia Jasminoides,Rhizoma Atractylodis Macrocephalae,Radix Bupleuri,Scutellaria Baicalensis,Radix Paeoniae Alba,Uncaria;Core prescription 5:Poria Cocos,Licorice,Pericarpium Citri Reticulatae,Radix Angelicae Sinensis,Peony Bark,Rhizoma Atractylodis Macrocephalae,Pinellia Ternata,Prunella Subtilis;Core prescription 6:Poria Cocos,Radix Paeoniae Alba,Radix Bupleuri,Cortex moutan,Rhizoma Atractylodis Macrocephalae,Gardenia Jasminoides,Licorice,Radix Angelicae Sinensis.Conclusion:(1)Professor Ma Jian often treats hyperthyroidism with tonifying deficiency drugs,clearing heat drugs,resolving phlegm,relieving cough and asthma drugs,promoting water and dampness,relieving exterior drugs and regulating Qi drugs.The core prescription drugs are Poria cocos,Atractylodes Macrocephala,Bupleurum,Peony bark,White peony root and Gardenia.(2)Professor Ma Jian treats hyperthyroidism mainly with bitter and sweet drugs,mainly with cold and warm drugs,which are mostly classified into lung meridians,spleen meridian,liver meridian and heart meridian.(3)In the course of treating hyperthyroidism,Professor Ma Jian proposed that emotion was the main cause,liver depression and spleen deficiency as the main pathogenesis,phlegm stasis as the main pathological product.(4)In the treatment of hyperthyroidism,Professor Ma Jian pays attention to the principle of dredging wood and reinforcing soil,as well as the principle of attack and supplement combined with application.He adopts the method of combination of dialectical and disease differentiation,with soothing liver and invigorating spleen,eliminating phlegm and removing blood stasis as the main therapeutic principles,supplemented by the methods of clearing heart fire and purging fire,relieving liver wind and nourishing yin and promoting fluid production,etc.
