| Objective: To observe and explore the clinical efficacy of needle warm moxibustion in prevention of lower extremity deep vein thrombosis(DVT)after ischemic stroke(IS),so as to provide a safe and effective prevention method for DVT of lower extremities in IS patients,in order to offer new clues and insights for clinical prevention and treatment of this disease.Methods: In this study,90 qualified patients were randomly divided into two groups(random number table method),including the acupuncture group and the needle warm moxibustion group.There are 45 patients in each group.Both groups received regular basic therapy of stroke.In addition,the acupuncture group patients were selected Biguan,Futu,Zusanli,Yanglingquan,Xuehai,Sanyinjiao and Taichong points on both sides and treated with conventional acupuncture therapy;based on the acupuncture group,needle warm moxibustion was performed at zusanli,Yanglingquan,Xuehai and Sanyinjiao points in the other group.Treatment was given to all participants once a day for half an hour.The whole course of intervention lasted for 3 weeks,with 6 consecutive days of treatment and 1 day of rest every week.Before and after intervention,1 month after intervention,3 months after intervention,patients of two groups were examined with color Doppler ultrasound and D-dimer(D-D),four blood clotting and hemorheology respectively.In this study,the relevant laboratory items,morbidity of DVT in lower extremities and incidence of slow blood flow in deep veins of lower extremities were regarded as observation indexes to analyze the intervention effect of the two therapies.Results:1.After 3 weeks of intervention,the morbidity of DVT and incidence of slow blood flow in deep veins of lower extremities in needle warm moxibustion group were lower than those in acupuncture group with statistical significance(p<0.05).1 month and 3 months after the intervention,the incidence of DVT of lower limbs in the warm acupuncture group was lower than that in the acupuncture group with statistical significance(p<0.05).1 month after the intervention,the incidence of deep vein slowing of lower limbs in the warm acupuncture group was lower than that in the acupuncture group,and the difference was statistically significant(p<0.05).2.After 3 weeks of intervention,the D-D,coagulation and hemorheology levels were improved in both groups,compared with those before intervention;needle warm moxibustion group’s index levels were better than acupuncture group’s,except TT,the difference was statistically significant(p<0.05).3.D-D,coagulation and hemorheology were compared after 1 month(follow-up 1)and 3 months(follow-up 2)of intervention within and between groups.All index levels in needle warm moxibustion group were improved at follow-up1 and 2,compared with those before intervention,except TT(p<0.05);all index levels of needle warm moxibustion group were better than those of follow-up after intervention,except TT,the difference had statistical significance compared with follow-up 2(p<0.05).All index levels of acupuncture group in the clinical follow-up were better than those before intervention,except TT,the difference between pre intervention and follow-up1 had statistical sense(p<0.05).All indexes in acupuncture group were better than those in follow-up 1 and 2 after intervention,except TT(p<0.05).All indexes of needle warm moxibustion group were better than those of acupuncture group at follow-up 1 and 2,except TT,the difference was statistically significant(p<0.05).Conclusions:1.Both needle warm moxibustion and conventional acupuncture are effective in preventing DVT of lower extremities after IS.The preventive effect and long-term curative effectiveness of needle warm moxibustion are better than conventional acupuncture.2.Needle warm moxibustion can regulate the coagulation function balance of patients with IS,alleviate the hypercoagulable state of blood,reduce blood viscosity and improve the venous circulation of lower limbs,so as to prevent the formation of lower extremity DVT. |
