| Objective:The purpose of this project is to explore the acupoint selection rules of acupuncture for post-stroke depression through data mining to verify the tutor’s theoretical basis of acupuncture prescription for the treatment of post-stroke depression.,Hoping to provide effective treatment methods for post-stroke depression and provide a theoretical basis for further research on acupuncture treatment,the observation on the curative effect in clinical trials has been done.Methods:1.By searching the literature on acupuncture treatment of post-stroke depression in various domestic databases,which have been published since the establishment of the database,summarizing and counting the frequency,location,meridian tropism and specific acupoint attributes of acupuncture and moxibustion,and the correlation analysis and cluster analysis were conducted.The acupoint selection rules of acupuncture treatment for post-stroke depression were discussed,which was mutually verified with the experience of tutor.2.66 patients with post-stroke depression were randomly divided into two groups who met the criteria for inclusion and exclusion.The treatment given to the treatment group combined acupuncture and moxibustion,the control group was given a placebo acupuncture combined with placebo moxibustion,and both groups were given conventional basic treatment at the same time.Acupuncture prescriptions were: acupuncture: GV 20,GV 24,GV29,PC 6(both sides),HT 7(both sides),LI 4(both sides),SP 6(both sides),LR 3(both sides);moxibustion: GV 20.Both groups were treated with two courses of treatment for a total of 12 times.Before and after treatment,HAMD-17 scale,BI scale and CSS scale were used to evaluate the improvement of depression,Activities of Daily Living and neurological function in the two groups.Results:1.Study on the law of acupoint selection: according to the inclusion criteria and exclusion criteria,480 articles were retrieved and 480 acupuncture prescriptions were collected,involving 145 acupoints.The top three acupoints were GV 20,LR 3 and PC 6;acupoints were mostly distributed in the head and face;governor vessel,bladder meridian and pericardial meridian were the most frequently used;the highest frequency of specific acupoints were intersection acupoints and five-shu acupoints.After correlation analysis,it was found that GV 20,as the core acupoint,had the highest correlation with other acupoints,and the combinations of "GV 20-LI 4-GV 29" and "LR 3-LI 4-GV 29" were often used.Cluster analysis showed that acupuncture prescriptions were mainly composed of acupoints from two angles: viscera syndrome differentiation,loss of nourishment of brain spirit and depression of spirit.2.Clinical study: a total of 66 participants were included,of which 2 in the control group withdrew from the study for personal reasons.In the end,33 cases were completed in the treatment group,31 cases were completed in the control group.Before treatment,in the baseline comparisons of age,sex,stroke type,course of disease,HAMD-17 scale,BI scale and CSS scale between the two groups was not significantly statistically different(P >0.05),and the two groups were comparable.After 12 treatments,the total effective rate of the treatment group was 84.8%,the control group was 51.6%.Compared within the group,the terms of HAMD-17 scale,BI scale and CSS scale compared with the pre-treatment scale(P<0.001)were statistically different;in the comparison between groups,the differences in the scores of HAMD-17 scale,BI scale and CSS scale before and after treatment reached statistical significance(P <0.05).Conclusion:1.By exploring the rule of acupoint selection for acupuncture treatment of post-stroke depression based on data mining,the following characteristics are found: focusing on local acupoint selection(mainly head),distal acupoint selection combined with proximal acupoint selection,acupoint selection along meridian,and attaching importance to the use of governor vessel and bladder meridian acupoints;intersection acupoints and five-shu acupoints are often selected;among all acupuncture prescriptions,GV 20 has the highest frequency and has a strong correlation with other acupoints.Acupuncture prescriptions are mainly composed of acupoints based on viscera syndrome differentiation,loss of nourishment of brain spirit and depression of spirit.Through data analysis,it is found that the acupoint selection rule of acupuncture in the treatment of post-stroke depression is highly similar to the acupuncture prescription selected in the clinical treatment of the tutor.2.Through the clinical study,it is proved that the acupuncture prescription is effective in the treatment of post-stroke depression,which can reduce the scores of HAMD-17 scale and CSS scale,increase the score of BI scale,regulate depression,improve the function of daily life and nerve function,and no obvious adverse reaction has been observed. |
