Objective To explore the evolution law of different stages of patients with chronic kidney disease is helpful to have a more comprehensive understanding of chronic kidney disease.1 Study 1: Literature study on TCM syndrome types of different stages of chronic kidney disease1.1 Methods Based on the relevant literature on chronic kidney disease.The CNKI database was searched for the time limit of building the database until December 31,2021.The search terms are TCM syndrome type,TCM syndrome type,chronic kidney disease,etc.To collect the literature screening,eliminate repeat,chronic kidney disease single stage,the results of the missing literature,the author of the literature information,research year,region,literature type,samples,patient gender,age,syndrome differentiation standard source,traditional Chinese medicine,establish a database.Meta-analysis was performed using Revman 5.4 software to summarize the TCM syndrome patterns of chronic kidney disease and its complications or comorbidities at different stages.1.2 Results TCM certificates of different stages of chronic kidney disease: a total of 21 documents were included in the study.Three of them are academic journals,and the rest are dissertations.The syndrome types of chronic kidney disease were both deficiency and reality,involving 13 syndrome types,of which 7 are deficiency syndrome and 6 are reality syndrome.In the deficiency syndrome type,spleen-kidney-Yang deficiency is the main in CKD1 stage,spleen-kidney qi deficiency is the main in CKD2-4 stage,spleen-kidney-yang deficiency is the main in CKD5 stage.CKD1 stage is dominated by dampness and turbidity syndrome,CKD2 stage is dominated by dampness and heat syndrome,ck D3-5 stage is dominated by dampness and turbidity syndrome.2 Study 2: Baseline characteristics of chronic kidney disease in the SPRINT study2.1 Methods Based on the medical records of the SPRINT research collected by the Biologic Specimen and Data Repository Information Coordinating Center of the National Heart,Lung and Blood Institute.Including clinical physical and chemical indicators as part of "microscopic syndrome differentiation",analyzing the baseline characteristics of patients with chronic kidney disease in the SPRINT study,and exploring the correlation of e GFR and other evaluation indicators in patients with chronic kidney disease.Statistical analysis was performed using the SPSS25.0 software.The statistical results were expressed as x±s for those conforming to the normal distribution,and independent sample t-test was used for comparison between groups.M(QR)for those not conforming to the normal distribution,Wilcoxon test was used to compare comparisons between groups;Analysis of the count data was performed using the chi-square test.P <0.05 was considered as a statistically significant difference.2.2 Results2.2.1 There were 9360 participants,6029 males and 3331 women,with a ratio of 1.8:1;the minimum age was 50 and 90 years,including 2636(28.16%).2.2.2 Age was positively associated with SBP,number of antihypertensive drugs,Framingham 10-year cardiovascular score,and high-density lipoprotein,and negatively associated with e GFR,DBP,cholesterol,and triglycerides.2.3.3 In the SPRINT study,CKD has 3337 individuals,including 2054 males and 1283 females,with a ratio of about 1.6:1;age ranged from 50 to 90,including 1415(42.40%).2.2.4 The e GFR of C K D population was about positive with SBP and 10-year cardiovascular score in Framingham,and negative with the number of antihypertensive drugs and triglycerides.3 Conclusion3.1 The essence of chronic kidney disease is the deficiency of the origin,and the evolution of syndrome type is as follows: in the early stage of chronic kidney disease,the deficiency syndrome of spleen and kidney mainly includes deficiency syndrome and other syndromes,combined with dampness and turbidness and dampness and heat;In the late stage of chronic kidney disease,deficiency syndrome such as deficiency of spleen and kidney still exists,but it is mainly positive,such as dampness and turbidity,blood stasis,and fierce evil poison.3.2 Patients with C K D have liver and kidney deficiency,dizziness,and hypertension.With the progression of renal function,the number of antihypertensive drugs applied increased,resulting in a mild decrease in SBP.This also suggests that we should be careful in the application of antihypertensive drugs in patients in advanced stages of CKD.3.3 CKD patients with "long disease into collateral and blood stasis",blood stasis and pulse,chest obstruction,heart failure,etc.Blood stasis and tendon loss,resulting in an increased risk of CVD.As CKD advances,blood stasis evidence gradually increases and the risk of CVD increases.3.4 The evolution of wet turbidity in C K D patients is the same as triglycerides with eGFR.
