Objective: To investigate the clinical application value of CT-guided micro-coil Positioning in thoracoscopic surgery for small Pulmonary nodules.Methods: This study is a retrospective study.The clinical data of 227 Patients with small Pulmonary nodules who underwent thoracic surgery in the Thoracic Surgery Department of the Affiliated Hospital of Guilin Medical College between August 2019 and August 2021 were collected.They were divided into a micro-coil localization group(145 Patients,158 nodules)and a non-localization group(82 Patients,84 nodules).The success rate of thoracoscopic wedge resection and thoracoscopic conversion to small incision were compared between the two groups.Thoracotomy,thoracoscopic wedge resection operation time,total operation time,surgical blood loss,Postoperative hospital stay,Postoperative thoracic drainage tube indwelling time;statistical analysis of CT-guided micro-coil Positioning success rate,Positioning time,Positioning complications Symptoms and other related indicators data to evaluate its efficacy and safety.Results: All 145 Patients in the Positioning group completed the Positioning of the micro-coil,and the Positioning time was(15.8±6.31)min.In terms of complications,19 cases(13.1%)of small amount of pneumothorax,15cases(3.4%)of intrapulmonary hemorrhage,and 2 cases(1.3%)of coils fell off.The micro-coil localization group and non-localization group were significantly different in gender(P=0.269),age(P=0.470),Previous history of malignant tumor(P=0.116),nodule-to-pleural depth(P=0.088),and total operation time(P=0.088).=0.230)and surgical blood loss(P=0.723),there was no significant difference;nodule composition(P<0.001),nodule lobe distribution(P=0.039),nodule maximum diameter(P<0.001),wedge resection operation time(P=0.015),thoracoscopic conversion to small incision thoracotomy(P=0.020),Postoperative thoracic drainage tube indwelling time(P=0.001),Postoperative hospital stay(P=0.030)differences statistically significant.Conclusions: Preoperative CT-guided micro-coil Positioning can improve the success rate of thoracoscopic surgery in the diagnosis and treatment of small Pulmonary nodules.The Positioning method is safe and reliable,with short hospital stay and minimal related complications.It is a Preoperative Positioning method worthy of Popularization and application.For all Patients Planning to undergo VATS with Pulmonary nodules that are not on the surface of the Pleura,especially those with less solidity and deeper locations from the Pleura,CT-guided microsprings should be considered before VATS.Circle Positioning.
