Objective:Investigate the intention to retain employment of rural order-oriented general practitioners after the expiry of the contract as well as its influencing factors in Hainan province,and analyze corresponding countermeasures,so as to improve the intention to retain employment of order-oriented general practitioners in Hainan Province,improve the employment status,and promote the development of primary medical care in Hainan Province.Methods:By using the cluster sampling method,this paper selected 238orders-directed on-duty general practitioners who graduated from Hainan Medical University during 2015-2017 periods as the survey objects.According to the results of pre-survey and expert evaluation,a self-made questionnaire was used as the survey tool and the form which was adopted to investigate the impact of factors such as the employment status of general practitioners with Hainan order orientation in post in grassroots medical institutions and intention to retain employment after the expiry,the location of unit,age,gender,marriage,whether being the one-child or having kids,culture degree,the department,job title,duties,monthly income,expected income,job satisfaction,the grassroots health service ability,and ability to continue to ascend on the general practitioners’ intention to stay after performance expiration.Results: A total of 238 questionnaires had been issued and 231 valid questionnaires were recalled.Only 62 out of 231 general practitioners(26.84 percent)were willing to stay on.As shown by single-factor analysis,the unit areas,age,monthly income,the feelings of job satisfaction of being a general practitioner,harmonious working environment,the aspiration to spend the future career as a practitioner in this unit,the basic work attitude,clinical diagnosis and treatment level of health service ability,and team cooperation spirit are associated with whether general practitioners are willing to continue to work.The difference was statistically significant(P<0.05).As shown by the regression analysis of binary logistics,the location of unit(OR=0.653,95%CI: 0.644-0.919),age(OR=0.292,95%CI: 0.101-0.841),monthly income(OR=2.067,95%CI: 1.188-3.597)and the aspiration to spend the future career as a practitioner in this unit(OR=3.838,95%CI: 2.393-6.156)were the influencing factors of whether rural orders-oriented general practitioners were willing to continue working(P<0.05).Conclusions: To conclude,rural orders-oriented general practitioners in Hainan province had the low intention to stay on after fulfilling their contract.In addition,the location of unit,age,monthly income and future medical career willingness were the influencing factors of whether general practitioners were willing to stay on after fulfilling their contract.In order to improve the retention rate of general practitioners after fulfilling their contract,the competent government departments need to enhance the incentive policies of general practitioners,improve the remuneration of general practitioners at the grass-roots level,improve the unbalanced development of different regions in Hainan Province,and strengthen the education of professional identity of general practitioners,and focus on the cultivation of general pratitioners’ professional identity is the only way to increase the retention rate of general pratitioners after completion of their performance.
