PurposeThe purpose of this study is to construct a scientific core competence evaluation index system suitable for the development level of neonatal intensive care unit(NICU)specialist nurses in China,and to lay a theoretical foundation for further deepening the curriculum content of NICU specialist nurse training and the construction of teaching staff and other related fields.It provides a scientific basis for the rational allocation of nursing human resources in the NICU,and provides an objective basis for managers to conduct assessments and evaluations.Methods1.In this study,the basic framework of the core competence evaluation index system for specialist nurses in the NICU was formed through the combination of literature review analysis and semi-structured interviews.Delphi method was used to conduct two rounds of correspondence,experts are invited to give their opinions on the content,score the importance of the indicators,and form an evaluation index system for the core competence of specialist nurses in the NICU.Calculating the positive coefficient,authoritative coefficient and coordination coefficient of experts was used to analyze the enthusiasm,authority and coordination of opinions of experts.The Delphi method is combined with the analytic hierarchy process(AHP)to determine the weight of each index and carry out the consistency test.2.Based on the constructed NICU core competence evaluation index system for specialist nurses,the NICU specialist nurses who meet the inclusion criteria in 8 tertiary A-level general hospitals in Shandong Province were a surveyed in a questionnaire from November to December 2021 to preliminarily test the reliability and validity of the index system.Results1.Results of semi-structured interviews: Through refining the interview content,the following six themes were obtained:(1)Solid theoretical knowledge of neonatal specialty(2)Skilled neonatal specialty practice ability(3)Scientific research capability that should be possessed(4))Ability to use critical thinking in work(5)Master relevant legal and ethical knowledge(6)Mastery of management skills.2.Expert enthusiasm degree: the degree of enthusiasm of experts is expressed by the recovery rate of the questionnaire.The first round was 93.75%,and the second round was100%.Among them,29 experts put forward constructive suggestions for revision of the indicators,indicating that the enthusiasm of consulting experts in this study is high,and the participation rate is good.3.Expert authority degree: The expert authority coefficient(Cr)is the arithmetic mean of the judgment basis coefficient(Ca)and the familiarity coefficient(Cs)of the correspondence experts for each indicator.It is 0.914 in the first round and 0.912 in the second round,which indicates that experts have a high degree of authority.4.Expert opinion coordination degree: The expert opinion coordination coefficient is expressed by Kendall’s harmony coefficient(ω),which ranges from 0 to 1.The larger the value of ω,the better the expert coordination coefficient.The minimum Kendall coordination coefficients of the first and second rounds were 0.334 and 0.421,respectively,and the χ2 test P<0.001.The results were statistically significant,indicating that the expert coordination degree was high,and the consultation results were desirable.5.Weight coefficients of indicators at all levels: the weight range of the first-level indicators is 0.1301-0.1502,the weights of the second-level indicators are 0.0082-0.0623,and the weights of the third-level indicators are 0.0010-0.0289.All passed the consistency test,indicating that the weight setting of each indicator is reasonable.6.Questionnaire survey results: A total of 330 questionnaires were distributed using the “Wenjuanxing” platform,with a recovery rate of 100%,excluding 10 invalid questionnaires,and 320 valid questionnaires,with an effective recovery rate of 96.97%.7.Reliability and validity analysis: Reliability coefficient(Cronbach α)= 0.942,calculated KMO(factor analysis),the result is equal to 0.918(P=0.000),the Cronbach’s αcoefficient of each dimension is between 0.732-0.865,the questionnaire scoring consistency is good.It shows that the evaluation index system we constructed has high consistency and stability.Validity analysis was carried out from content and construct validity,and the results showed that this study has high accuracy and validity.ConclusionIn this study,literature review,semi-structured interviews,correspondence from Delphi experts,and AHP finally formed an evaluation index system for the core competence of NICU specialist nurses,including 7 first-level indicators,26 second-level indicators,and 133 third-level indicators.In the 2 rounds of expert consultation,the enthusiasm coefficients of experts were 93.75% and 100%,and the authoritative coefficients were 0.914 and 0.912.The enthusiasm,authority and coordination of opinions of the consulting experts were good,and the opinions of experts were highly consistent.,The weight value of each indicator(first-level,second-level and third level)is set reasonably via AHP.Secondly,the reliability and validity of the indicators are good,thus ensuring the scientificity and credibility of the research results,laying a theoretical foundation for the training of specialist nurses in the NICU and the construction of the teaching staff,and for the nursing human resources of the NICU.It provides a scientific basis for the rational allocation and ability evaluation of the NICU nursing. |
