Objective:To explore the application of double filtration plasmapheresis(DFPP)and hemoperfusion(HP)in the treatment of patients with hyperlipidemic pancreatitis(HLAP)and to improve the level of hematology in our province. To improve the technical level of blood purification in our province. Methods:Clinical data of a total of 42 cases each in the DFPP and HP groups of HLAP patients in Xining area(Qinghai Provincial People’s Hospital and three other hospitals)from January 2018 to November 2021 were collected strictly according to the inclusion and exclusion criteria, including:general data(gender, age, ethnicity),vital signs, pancreatic function, blood lipids, liver and kidney function electrolytes, inflammatory indexes, coagulation, blood gas and APACHCEII score, etc. We compared the changes of each of the above indicators before and after treatment in the two groups, respectively, and analyzed the differences in the efficacy of the DFPP and HP groups in HLAP treatment.Results:1.The differences in basic information between DFPP and HP groups were not statistically significant;2.intra-group comparison between DFPP and HP patients before and after treatment:(1)the differences in the expression of TG,TC,LDL-C,HDL-C,AMY,PAMY,Ca,APACHEII score,CRP,AST,ALT,TBIL,ALB,Cr,urine volume, and INR in both groups were statistically significant,P≤0.05;(2)in the DFPP group, the differences in LIP were statistically significant,P(DFPP)≤0.05;(3)in the HP group, the differences in Glu,APTT were statistically significant,P(HP)≤0.05;3.Comparison between groups:the differences in TG,TC,AMY,APACHEII scores were statistically significant in the DFPP group,P(DFPP)≤0.05.Conclusions:1.Both DFPP and HP were effective in reducing lipids, inflammatory indexes, pancreatic enzymes, improving liver and kidney function, and rapidly improving the condition, both of which can be used in the treatment of HLAP,consistent with the literature.2.HP is less effective than DFPP in regulating lipids and pancreatic enzymes and improving overall efficacy.3.APTT and Glu were significant before and after treatment in HP,and LIP was significant before and after treatment in DFPP The results of the two groups were not completely consistent.4.One study reported a trend toward decreased local and systemic complications between patients with TG≥11.30 mmol/L compared with those with TG between5.65 mmol/L and 11.30 mmol/L,but the difference was not statistically significant. The results of the present study are generally consistent with this report.
