A Comparative Study Of CLIF Combined With Wiltse Approach Pedicle Fixation And TLIF In The Treatment Of Degenerative Lumbar Scoliosis

【Objective】To investigate the short-term efficacy of CLIF combined with Wiltse approach for pedicle fixation in the treatment of Degenerative Lumbar Scoliosis,and find out the advantages and disadvantages of this operation in the treatment of Degenerative Lumbar Scoliosis comparing with the traditional TLIF operation.【Methods】Data of 51 patients with degenerative lumbar scoliosis admitted to our hospital from June 2019 to June 2021 were retrospectively analyzed.Among them,27 patients were treated with CLIF combined with Wiltse approach pedicle fixation(CLIF group),and 24 patients were treated with TLIF surgery(TLIF group).Record the perioperative data of 51 patients: Age,Course Cf Disease,Sex,Operation Section,Operative Blood Loss,Operative Time,Postoperative Hospital Stay;imaging changes: preoperative and postoperative Lumbar Lordosis(Lumbar Lordosis,LL),Corona Cobb Angle(Corona Cobb Angle,CCA),Pelvic Tilt(Pelvic Tilt,PT),Disc Height(Disc Height,DH),Foramen Height(Foramen Height,FH),Coronal Vertical Axis(Coronal Vertical Axis,CVA),Sagittal Vertical Axis(Sagittal Vertical Axis,SVA);function: preoperative,postoperative,and follow-up Visual Analog Score(Visual Analog Score,VAS),Oswestry Dability Index(Oswestry Dability Index,ODI);complications and fusion rates.Finally,the data were statistically analyzed.【Result】Intra-group comparison,CCA,LL,DH,FH,PT,CVA,SVA,VAS,and ODI were significantly improved after surgery,the difference was statistically significant(P<0.05).There were significant differences in VAS,ODI between the last follow-up and the postoperative(P<0.05).Comparison among groups,There was no statistical difference between groups(P>0.05).in preoperative Age,Course Of Disease,CCA,LL,DH,FH,PT,SVA,CVA,VAS and ODI.CCA,LL,DH,FH were statistically different between the two groups at the postoperative(P<0.05),while PT,SVA,CVA at the postoperative and VAS,ODI at the last follow-up and postoperative were not statistically different between the two groups(P>0.05).The amount of blood loss in CLIF group was less than that in TLIF group,and the difference was statistically significant(P<0.05).The length of postoperative hospital stay and operation time were not significantly different between the two groups(P>0.05).【Conclusion】CLIF combined with Wiltse approach for pedicle fixation has clear short-term efficacy in the treatment of Degenerative Lumbar Scoliosis.Compared with TLIF,the operative blood loss is less,and the correction effect in local coronal and sagittal malformations of the spine is better.
