Objective1.In this study,we applied the simplified "six-character recipe" of Ziwu Liujiao to the elderly patients with COPD in the stable stage of lung spleen qi deficiency and investigated its effect on their pulmonary rehabilitation.2.The aim is to provide patients with an effective,economical,simple and highly acceptable TCM care technique and to enrich the TCM care program for pulmonary rehabilitation through standardized scientific research to verify the effectiveness,possibility and safety of the "six-character recipe" to improve patients’ pulmonary rehabilitation.MethodsThe study was designed according to a randomized controlled trial,with strict inclusion and exclusion criteria.122 patients were selected from June 2021 to April 2022 who were admitted to the respiratory department and geriatric department of a tertiary care general hospital in Hangzhou and diagnosed as elderly patients with COPD in the stable stage of lung spleen deficiency by diagnostic criteria.Group A was given conventional treatment and care;Group B was given the traditional six-character recipe on the basis of Group A;Group C was given the simplified "six-character recipe" on the basis of Group A;Group D was given the simplified "six-character recipe" on the basis of Group A.The intervention periods were The intervention periods were before,4 weeks after and 12 weeks after the intervention.Pulmonary function,6-min walking distance,dyspnea index and Chronic Obstructive Pulmonary Disease Assessment Test Scale(CAT)scores were evaluated before,4 weeks after and 12 weeks after the intervention,respectively.Results1.Completion of the experiment in four groups of patients:128 patients who met the study criteria were included in this study,and 122 cases were finally completed,of which 31 cases were completed in group A and 2 cases were shed;30 cases were completed in group B and 3 cases were shed;30 cases were completed in group C and 3 cases were shed;31 cases were completed in group D and 2 cases were shed.2.Comparison of the four groups of patients before the intervention at baseline:as shown by the baseline data of the enrolled patients,the comparison of the data of patients’gender,age,height,disease duration,weight,BMI index,and smoking cessation showed no statistical significance(P>0.05).The comparison of patients’ pulmonary function,6-min walking distance,dyspnea index and CAT score before the intervention showed no statistically significant differences(P>0.05),which showed that the general information was consistent at baseline and the four groups were comparable.3.Pulmonary function indexes:Group A had no significant changes in pulmonary function indexes before,4 weeks after and 12 weeks after intervention,except for FEV1 indexes which had changes compared with those before intervention after 12 weeks of intervention;Group B had no significant changes compared with those before intervention after 4 weeks of intervention,and groups B,C and D had significant changes in various indexes of pulmonary function after 4 weeks of intervention and 12 weeks of intervention compared with those before intervention;between the same groups The lung function indexes in groups B,C and D showed significant changes at 12 weeks after the intervention compared with 4 weeks after the intervention,except for group A,where there were no significant changes in lung function indexes,and groups B,C and D,where there were significant changes at 12 weeks after the intervention compared with 4 weeks after the intervention;group C showed significant changes compared with group A and group D compared with group A at the same time point after 12 weeks after the intervention.4.Activity tolerance index(6-minute Walking Distance,6MWD):Patients in group A had no significant changes in their 6MWD after 4 weeks and 12 weeks of intervention;groups B,C and D had significant changes after 4 weeks and 12 weeks of intervention compared with before intervention,group C was higher than group B and group D was better than the other two groups;groups B and D had significant changes between the same groups after 12 weeks of intervention compared with after 4 weeks of intervention;group C had significant changes compared with group A after 4 weeks and 12 weeks of intervention;group D had significant changes compared with group A after 4 weeks and 12 weeks of intervention.There were significant changes in group C compared with group A after 4 weeks and 12 weeks of intervention;there were significant changes in group D compared with group A after 4 weeks and 12 weeks of intervention.5.Dyspnea index(Modified British Medical Research Council,mMRC):Patients in group A had no significant changes in their mMRC after 4 weeks and 12 weeks of intervention;except for group B,which had no significant changes after 4 weeks of intervention compared with the pre-intervention period,all had significant changes;groups B,C and D had significant changes at 12 weeks after intervention compared with 4 weeks after intervention;group C had significant changes compared with group A and group D compared with group A at the same time point after 12 weeks of intervention.6.Quality of life index(Chronic Obstructive Pulmonary Disease Assessment Test Scale,CAT):There were no significant changes in CAT scores in group A at 4 weeks and 12 weeks after the intervention;there were significant changes in group B,C and D at 4 weeks and 12 weeks after the intervention compared with the pre-intervention period;CAT scores in group B>CAT scores in group C>CAT scores in group D;there were significant changes in group C and D at 12 weeks after the intervention compared with 4 weeks after the intervention;there were significant changes in group C compared with group A and group D compared with group A at the same time point after 12 weeks after the intervention.Conclusion1.The simplified "six-character recipe" of Ziwu Liujiao is conducive to improving the pulmonary rehabilitation function of elderly patients with COPD in the stable stage of pulmonary spleen qi deficiency,and the longer the intervention time,the better the effect than the other two methods.2.The addition of the traditional six-character recipe or the simplified "six-character recipe" to conventional treatment and care showed some improvement in the 6WMD,mMRC and CAT scores of the patients,but the longer the intervention time of the simplified "sixcharacter recipe" was chosen,the more significant the care effect was.3.The intervention program adopted in this study is safe and effective,simple and easy to learn,not restricted by the site,and highly acceptable to patients,which is worthy of application and promotion in clinical care and community care.
