ObjectiveThrough the questionnaire survey to understand stability of chronic obstructive pulmonary disease(COPD)patients with inhalation medication compliance status and influencing factors,adopts qualitative research from the view point of patients with long-term use of inhaled drugs promote and hinder factors,stability for the targeted management of COPD patients,improve inhaled medication adherence to provide theoretical basis.Methods1.Using the convenience sampling method,from September 2020 to May 2021,213 patients who met the inclusion criteria were selected for outpatient treatment of chronic diseases in the respiratory department of a tertiary hospital in Zibo City for questionnaire survey.Research tools include:General Information Questionnaire,Modified British Medical Research Council Dyspnea Questionnaire,Inhalation Treatment Compliance Questionnaire in Patients with Chronic Airway Disease,COPD Health Literacy Questionnaire,and Social Support Scale.SPSS 21.0 software was used for statistical analysis of the data,the frequency and composition ratio were used to describe the general data of the research objects,the mean±standard deviation was used to represent the current status of inhalation medication compliance,and univariate analysis and multiple linear regression analysis were used to explore the stable period The influencing factors of inhalation medication compliance in COPD patients were statistically significant at p<0.05.2.Semi-structured in-depth interviews with 22 patients with long-term inhalation drugs were conducted,and the data were analyzed using the Colaizzi phenomenological seven-step analysis method.Results1.The total compliance score of inhaled medication in patients with stable COPD:(80.29±7.32)points.The scores of each dimension:dose compliance(43.63±6.13)points,inhalation technical specifications(36.66±3.75)points,and the compliance status is at a medium level.2.Univariate analysis showed:gender,age,place of residence,educational level,occupation,work status,type of medical insurance,per capita monthly income of the family,smoking,duration of inhalation device use,types of inhaled drugs used,acute There was an association between the number of exacerbations,mMRC grade and compliance with inhaled medication(p<0.05).Age,marital status,place of residence,living style,occupation,work status,type of medical insurance,per capita monthly household income,duration of COPD,smoking,duration of inhalation device use,type of inhalation device used and dose compliance sex(p<0.05).Age,marital status,living style,educational level,occupation,work status,per capita monthly income of family,duration of COPD,duration of inhalation device use,number of acute exacerbations,mMRC classification and inhalation technical specifications were correlated(p<0.05).3.Multiple linear regression analysis showed that:COPD health literacy,the number of acute exacerbations in the past year,and social support factors could explain 37.5%of the patients’ compliance variables for inhaled medication.COPD health literacy,mMRC classification,COPD duration,availability of support,and marital status could explain 47.3%of patients’ dose compliance.Age,mMRC classification,social support,living style,and the number of exacerbations in the past year could explain 39.6%of the patients’ inhalation technical specifications.4.Six themes were obtained through qualitative interviews:improvement of life quality,gratitude,change of self-management ability,lack of knowledge,great economic pressure and fluke psychology.Conclusions1.The overall compliance of inhaled medication in stable COPD patients is at a medium level.2.The main influencing factors of inhaled drug treatment compliance in stable COPD patients were COPD health literacy,the number of exacerbations in the past year,and social support.3.Influencing factors of various dimensions of inhalation medication compliance in stable COPD patients:COPD health literacy,mMRC classification,COPD course,a vailability of support,and marital status mainly affect dose compliance.Age,mMRC classification,social support,living style,the number of acute exacerbations in the past year,and educational level mainly affected the inhalation technical specifications.4.The promoting factors affecting patients’ compliance with inhaled drug administration include improved quality of life,psychological thanksgiving,improve self management ability,hinder factors include lack of knowledge,the economic pressure of drugs and some patients exist fluky psychology,suggesting that medical staff should p ay attention to patients’ use experience and provide multi-dimensional support strategies.
