Objective:1.To systematically evaluate the effectiveness and safety of orthokeratology and frame glasses for prevention and control of myopia in adolescents;2.To systematically evaluate the effectiveness of low concentration atropine combined with rimmed glasses and rimmed glasses on prevention and control of myopia in adolescents.Methods:1.CNKI and WanFang Data databases were searched, and Chinese search terms were orthokeratology, frame glasses and myopia,The search time was up to 2022-12.Randomized controlled trials comparing the prevention and control effects of orthokeratology and glasses on myopia in adolescents were included. The improved JADAD score scale was used to evaluate the quality of the included literature.Meta analysis was performed using Stata15 software.Sensitivity analysis and subgroup analysis were used to find the source of heterogeneity, and funnel plot was used to detect publication bias in the included literature.2.Search CNKI,WanFang Data database,Chinese search terms:atropine, myopia, diopter, axis of eye.Search time to 2023-1.The literature on the comparison of prevention and control effect of low concentration atropine combined with spectacles and glasses with frames on myopia was included. The quality of the included literature in the randomized controlled experiments was evaluated by the modified JADAD scoring scale, and the quality of the included literature in the non-randomized controlled experiments was evaluated by the MINORS scoring.Meta analysis was performed using Statal5 software.Sensitivity analysis was used to find the source of heterogeneity, and funnel plot was used to detect publication bias in the included literature.Results:1.There were 11 literatures on the comparison of the prevention and control effects of orthokeratology and glasses on myopia in adolescents, including 1286 subjects. The results of meta-analysis showed that:7 literatures were followed up for 1 year, and the results showed that the difference of axial changes between the orthokeratology group and the frame lens group was WMD=-0.362,95%CI:-0.533~-0.190,P=0.000,and the difference was statistically significant. Two studies were followed up for 2 years, and the results showed that the difference of axial changes between the orthokeratology group and the frame group was WMD=-0.640,95%CI:-1.169~-0.111,P=0.018,and the difference was statistically significant.Three literatures were followed up for 3 years, and the results showed that the difference of axial changes between the orthokeratology group and the frame lens group was WMD=0.290,95%CI:-0.541~1.120,P=0.494,and the difference was not statistically significant.A total of three literatures studied the number of adverse reactions in the orthokeratology group and the frame group. The results showed that after 1 year of follow-up,the RR of adverse reactions in the orthokeratology group and the frame group was RR=3.463,95%CI:0.259-46.363,P=0.348,and the difference was not statistically significant.2.There were 7 literatures on the comparison of the prevention and control effect of low concentration atropine combined with rimmed glasses and rimmed glasses on myopia, including 643 subjects.Among them,6 literatures studied 0.01%atropine eye drops combined with frame glasses and simple frame glasses, and 1 literatures studied 0.005%atropine eye drops combined with frame glasses and simple frame glasses. The results of meta-analysis showed that after 1 year of follow-up,the difference of axial changes between the low-concentration atropine combined with glasses group and glasses group was WMD=-0.192,95%CI:-0.296~-0.088,P=0.000,and the difference was statistically significant. The difference of diopter between the low concentration atropine combined glasses group and the frames group was WMD=-0.304,95%CI:-0.506~-0.103,P=0.003,and the difference was statistically significant.Conclusions:1.Compared with frame glasses, orthokeratology goggles can effectively control the progression of myopia in adolescents, and is relatively safe.2.Compared with simply wearing glasses, atropine combined with glasses with low concentration can effectively control the development of myopia in teenagers. |
