| Objective:To explore the factors leading to the result of spontaneous reduction and ultrasound-guided hydrostatic reduction(UGHR)in children with ileocolic intussusception and to analyze the prediction ability of these factors,raising the awareness of them and providing the basis for clinical decision making.Methods:The general data,biochemical examination,and ultrasonography of 155 children with ileocolic intussusception were retrospectively analyzed.Then divided into the spontaneous reduction group and the UGHR group,and the two groups were divided into two subgroups:the successful group and failed group,respectively.Monofactor analysis was used to determine the number of factors highly correlated with the outcome.Independent factors with statistical significance were analyzed using binary logistic regression analysis,and then the prediction ability was further evaluated by the receiver operating characteristic curve(ROC curve).Results:(1)There were 155 cases in the spontaneous reduction group,including 22 cases in the successful group and 133 cases in the failed group.Monofactor analysis showed that there were statistically significant differences in lymph node distribution,clinical symptoms,temperature,age,and outer rim thickness of the target between the two groups(P<0.05).Binary logistic regression analysis showed that the age and outer rim thickness of the target had statistical significance between the two groups(P<0.05).ROC curve analysis showed that the area under the curve(AUC)of outer rim thickness of the target and combined index were 0.65 and 0.68(both P<0.05),and their cut-off values were 0.74 cm(specificity 71.40%,sensitivity 56.90%)and 2.51(specificity 57.10%,sensitivity 80.80%),respectively.(2)There were 133 cases in the UGHR group,including 128 cases in the successful group and 5 cases in the failed group.Monofactor analysis showed that age and body mass index(BMI)were statistically significant differences between the two groups.Binary logistic regression analysis showed that the two were not independent factors influencing UGHR outcome(P>0.05).Conclusions:(1)Part of ileocolic intussusception is prone to reduction spontaneously.Age>3 years and outer rim thickness of target ≤0.74cm are the influencing factors for promoting the spontaneous reduction of intussusception in children,however,the prediction ability of outcome is limited.(2)UGHR has a high reduction success rate and broad indications,age and BMI were correlated with UGHR outcome,but they were not independent factors. |
