| Objective:To explore the potential relationship between leucine-rich α2glycoprotein 1(LRG1)and type 2 diabetic microangiopathy,to provides a theoretical basis for the role of LRG1 in the occurrence and development of T2DM microangiopathy,and to analyze the risk factors related to type 2 diabetic microangiopathy,so as to provide new ideas for the diagnosis,treatment and prognosis of patients with type 2 diabetic microangiopathy.Methods:103 T2DM patients who were hospitalized in the Department of Endocrinology of Subei People’s Hospital of Jiangsu Province from March 2021 to July 2022 and met the inclusion criteria were selected as the research objects.All patients were examined for diabetic microangiopathy,and were divided into T2DM microangiopathy group(group A)with 64 cases and simple T2DM group(group B)with 39 cases.In addition,35 healthy examinees matched with the age and sex of the experimental group were selected as the healthy control group(group N).General clinical data were collected from all subjects,and fasting venous blood was collected,use automated chemical analysis technology to detect fasting glucose(FPG),fasting insulin(FINS),fasting C-peptide(FCP)and blood lipid levels in blood samples,detection of glycated hemoglobin(HbA1c)by high performance liquid chromatography,urine analyzer to detect urine microalbumin-to-creatinine ratio(UACR),and serum samples were collected.The expression of LRG1 was detected by enzyme linked immunosorbent assay(ELISA),and insulin resistance index was evaluated by improved steady-state model(HOMA2-IR).All the collected data were processed by SPSS26.0 statistical software of IBM in the United States.One-way ANOVA or T test or nonparametric rank sum test or chi-square test was used to compare general data,laboratory indicators,LRG1 between groups.Pearson or Spearman correlation analysis is employed to assess the association between LRG1 and clinical and laboratory indices.Binary analysis was employed to evaluate the link between microangiopathy and risk factors in type 2 diabetes mellitus.The diagnostic efficacy of LRG1 and other indicators in the diagnosis of microangiopathy in T2DM was evaluated through logistic regression,utilizing the Receiver Operating Characteristics Curve(ROC).P<0.05 means that the difference is statistically significant.Results:1.No statistically significant distinction in gender between the three groups was observed(P=0.418),there was a statistically significant difference in systolic blood pressure between the three groups(P<0.05).However,age,BMI and diastolic blood pressure of group A and B increased when compared to group N(P<0.05).The course of disease in group A was longer than that in group B(P<0.05).2.FINS,FCP,TC,LDL-C and HOMA2-IR among the three groups had no statistical difference(P>0.05),there were differences between TG and HDL-C between the three groups(P<0.05).Group A and B’s FPG was greater than that of Group N(P<0.05).In group A,HbAlc was higher than that in group B(P<0.05).3.The level of LRG1 in female group was significantly higher than that in male group(P<0.05).There was a statistically significant difference in LRG1 levels between the three groups(P<0.05).4.The level of LRG1 was divided into groups from low to high by quartile method.The prevalence of type 2 diabetic microangiopathy with high LRG1 level group was significantly higher than that in the other three groups,and the difference was statistically significant(P<0.05).5.The level of LRG1 is positively correlated with age,BMI,FPG,FINS,TG,systolic blood pressure,diastolic blood pressure and HOMA2-IR,and negatively correlated with HDL-C.6.Course of disease,HbAlc and LRG1 are independent risk factors of type 2 diabetic microangiopathy(P<0.05).7.Analysis of the ROC curve revealed that the AUC of LRG1,course,HbA1c,and LRG1 in the diagnosis of type 2 diabetic microangiopathy was 0.881,surpassing the single index,suggesting that LRG1,course,and HbAlc had a high efficacy in diagnosing the condition.Conclusion:1.The expression of serum LRG1 in those with type 2 diabetic microangiopathy is augmented,and the prevalence of the condition is notably augmented in the group with a high LRG1 level.LRG1 is related to type 2 diabetic microangiopathy.2.The level of LRG1 is related to sex,age,BMI,FPG,FINS,TG,systolic blood pressure,diastolic blood pressure,HOMA2-IR and HDL-C.3.Course of disease,HbA1c and LRG1 are independent risk factors of type 2 diabetic microangiopathy.4.LRG1 is expected to be a potential new biomarker for the diagnosis of type 2 diabetic microangiopathy,and the combination of course,HbAlc and LRG1 has high diagnostic efficiency for type 2 diabetic microangiopathy. |
