Objective The purpose of this study is to explore the self-management experience and feelings of patients with radical cystectomy and urinary stoma during the transitional period after operation,to understand the problems and needs in the process of self-management during the transitional period,and to find out the factors affecting the self-management of patients during the transitional period after operation,so as to provide reference for carrying out targeted health education,formulating effective nursing intervention strategies during the transitional period after operation,and constructing continuous nursing programs for patients with radical cystectomy and urinary stoma during the transitional period after operation.Methods Using descriptive qualitative research methods,through purposive sampling,according to the maximum difference strategy,patients who underwent radical cystectomy and urostomy from the urology department of a tertiary hospital in Hangzhou were selected as the research objects.Semi-structured interviews were used to collect data and analyze the interview data.The data analysis was carried out according to the traditional content analysis method,including coding,classifying and refining the theme,and the analysis was carried out until the theme was saturated and no new theme was generated.Results In this study,a total of 16 patients with radical cystectomy and urinary stoma were interviewed,and the following five themes and 20 sub-themes were formed after sorting out and analyzing the data:1.Congnition of self-management in transition period:self-management in transition period is not necessary、self-management in transition period is its own thing、self-management in transition period needs family assistance、self-management in transition period is not so difficult;2.Transition period self-management motivation:Expect to achieve self-care、expect to return to work、expect to reduce social restrictions;3.Transition period self-management concerns:worry about security risks、worry about the waste of stoma supplies、worry about the body can not eat;4.Difficulties in self-management during the transition period:lack of self-management knowledge、lack of relevant nursing skills、difficulty in changing living habits、poor self-emotional regulation、poor role adaptation、and difficulty in managing sexual life;5.Transition period self-management reflection:learning in frustration、actively facing life、adhering to good behavior habits、living and learning.The factors affecting the self-management of patients during the postoperative transition period mainly include living environment factors,individual factors and self-management support factors.Conclusion Self-management cognition and disease-related knowledge and disease experience in patients with radical cystectomy during the postoperative transition period.In the early stage of the postoperative transition period,patients had wrong self-management cognition and weak self-management awareness.Patient motivation and concerns coexist in the process of self-management,which influence the choice of patient self-management mode.There are many difficulties in transitional self-management for patients,and patients are eager to get support and help from their family,society and medical staff.In the transition period of self-management,some patients can develop positive self-management methods and attitudes through self-reflection.The self-management behavior during the postoperative transition period is influenced by living environment,individual and supportive factors.
