| Objective: To study the effect of glucolipid metabolism of patients with the early s diabetic kidney disease(DKD)based on the platform of Metabolic management center(MMC)in order to provide suggestions for early detection,diagnosis and treatment of diabetic kidney disease.Methods: A total of 100 patients of matched condition and received from the First People’ s Hospital of Huaihua from May 2020 to December 2020;A total of 93 cases were finally included in the study,were selected and divided observation group and control group into 47 cases and 46 cases,respectively.and the rate of missing visit was7 %.The control group was treated with traditional intervention model.The observation group was treated with MMC platform intervention based on the control group and followed up for 180 days.The variations between the two groups before and after interference of fasting plasma glucose(FPG),2h postprandial glucose(2h PG),glycosylated hemoglobin(Hb A1c),triglyceride(TG),total cholesterol(TC),estimated glomerular filtration rate(e GFR),and urinary albumin-to-creatinine ratio(UACR)were compared.Results: 1 Comparison of the baseline data: the general data of the two group had not statistical significance(P > 0.05)and the two groups were comparable.2.The variations of FPG,2 h PG and Hb A1 c with time in the observation group and the control group were compared.The results showed that the test results of FPG,2h PG,and Hb A1 c of the two group patients declined with time(P < 0.05)and this trend in the observation group was greater than that in the control group.3.The comparison of TG changes before and after interference for 6 months showed that the t-test results of paired samples showed that both groups declined after 6months interference.Both groups showed a downward trend after 6 months of intervention,and the decline was greater in the observation group.The difference had statistical significance(P < 0.05).Comparison of TC changes before and after intervention for 6 months showed that both groups declined after 6 months interference.The difference of the observation group was statistically significant,but the control group was not statistically significant(P > 0.05).4.The changes of UACR with time in the observation and the control groups were compared.The results showed the UIACR of patients of two group declined with time and the observation group was greater than the control group.The results of time point comparison about controlling the grouping factors showed that there had no significant difference between the two group before treatment.After 3 months and 6 months interference,the observation group was lower than the control group and the difference was statistically significant(P < 0.05).5.The changes of e GFR with time in the observation and the control group were compared.The results showed the e GFR of patients of two group increased with time and the observation group was greater than the control group.Further simple effect analysis,the results of time point comparison about controlling the grouping factors showed that there had no significant difference between the two group before treatment.After 3 months and 6 months interference,the observation group was lower than the control group with and the difference was statistically significant(P < 0.05).6.Person correlation analysis was used to correlation analysis between Hb A1 c 、e GFR and UACR.The results showed that: without grouping before interference,there was a positive correlation between Hb A1 c and UACR,but there was no significant correlation between Hb A1 c and e GFR(P > 0.05).Conclusions: 1.Basing on the MMC interference can greatly improve Hb A1 c,FPG,2h PG and blood lipid levels of DKD patients.2.Basing on the MMC interference can increase e GFR and decrease UACR in DKD patients.3.The Hb A1 c levels were positively correlated with UACR in DKD patients. |
