Rearch On Health Management Model Of Cardiovascular Disease In Hospital-community-home Linkage

Cardiovascular diseases have become the primary threat to the life,health and quality of life of Chinese residents.Since the 21st century,the mortality rate,morbidity rate and morbidity rate of cardiovascular diseases have increased year by year,bringing huge social and economic burden to Chinese society and families.Therefore,it is very important to conduct healthy management of cardiovascular diseases and avoid premature end of life due to cardiovascular diseases.In terms of national policies,the Healthy China Action(2019-2030)proposes to take effective intervention measures to advance cardiovascular disease management,improve the efficiency of cardiovascular disease management and the quality of grass-roots health services as strategies,and community health institutions as the "main battlefield" of cardiovascular disease management.80-90%of cardiovascular patients should be placed in community health services for long-term continuous health management."Hospital-community-home integrated cardiovascular disease health management mode" refers to the whole-process health management of patients with cardiovascular and cerebrovascular diseases through the cooperation of hospital,community and home,so as to reduce the recurrence and mortality and improve the quality of life."Hospital-community-home combined cardiovascular disease health management mode" can provide a full range of management services,including medical history collection,physical examination,laboratory examination,drug management,lifestyle intervention,psychological support and other aspects.Through the cooperation among hospital,community and home,a perfect patient file and management system can be established to manage and intervene in the whole process of cardiovascular and cerebrovascular disease,so as to effectively control the progression and occurrence of the disease."Hospital-community-home linkage cardiovascular disease health management mode"can also improve the health literacy and health awareness of patients and their families,promote patients to actively participate in self-management,form good health habits and lifestyle,so as to reduce the occurrence and recurrence of cardiovascular and cerebrovascular diseases."Hospital-community-home combined cardiovascular disease health management mode" can effectively prevent and control the occurrence and progress of cardiovascular and cerebrovascular diseases,improve the quality of life and health of patients.In order to explore the problems in the process of hospital-community-home combined cardiovascular disease health management in X district,this study selected a community medical and health service institution in X district,Y city for investigation,to understand the theories related to health management and the research status of cardiovascular disease health management at home and abroad.By comprehensive use of data collection and data analysis methods,the operation of the hospital-community-home-based linkage cardiovascular disease health management mode was clarified,the difficulties of the current hospital-community-home-based linkage cardiovascular disease health management mode were screened,and the causes of the hospital-community-home-based linkage cardiovascular disease health management mode were explored.Based on the investigation,analysis and comparative study,it is concluded that there are still some problems in the process of hospital-community-home combined cardiovascular disease health management in X district:(1)The level of community medical care is low,and the degree of trust of patients is not high;(2)Lack of linkage between hospitals and communities,and lack of control by other institutions;(3)Serious brain drain and imperfect management system;(4)The different levels of residents’ health literacy affect the efficiency of health management.The main reasons are:(1)Uneven allocation of health resources and insufficient cardiovascular funding;(2)Internal and external coordination service is not smooth,linkage mechanism has not been built;(3)Lack of unified efficient management system,interconnection and interoperability is insufficient;(4)Residents lack health awareness and health education is not deep enough.Based on the above problems and reasons,this paper puts forward the following countermeasures:(1)Use modern information technology to improve medical efficiency and strengthen the training of health management related talents;(2)Improve internal and external coordination services and strengthen the construction of intensive medical association;(3)Give full play to the macro-control role of the government,optimize the talent management system;(4)Build an efficient management system and establish an interconnection platform;(5)Increase the popularization of health publicity and comprehensively improve the level of health literacy.
