| Objective To explore a set of convenient and accurate methods for intraoperative positioning of prosthesis by using the limited instruments provided by the device manufacturers.And the short-term clinical efficacy analysis of the patients after using this method,and compared with the total knee arthroplasty group.Method Our study is a retrospective study.Based on previous surgical experience,a set of methods for osteotomy,positioning and prosthesis installation using existing instruments has been explored.The inaccurate methods such as touching the center of the femoral head or the anterior superior iliac spine in the previous operation were abandoned.Combining the principle of osteotomy in total knee arthroplasty,the method of reference osteotomy guide fixed needle was used to improve the accuracy and speed of placement of femoral and tibial prosthesis.The 82 patients in the unicompartment knee arthroplasty group and 84 patients in the total knee arthroplasty group with the classical surgical scheme were compared,and the differences between the two groups in terms of operation time,bleeding volume,joint range of motion,visual analogue score(VAS),forgotten joint score(FJS),hospital for special surgery(HSS)and postoperative complications were analyzed.Results Postoperative X-ray showed that the position of the prosthesis was appropriate,and the short-term effect of the patient was reliable.Compared with the total knee arthroplasty group,the UKA group had higher visual analogue score(VAS),forgotten joint score(FJS),hospital for special surgery(HSS),shorter operation time and less intraoperative bleeding,and the difference was statistically significant.There was no statistical difference in postoperative complications and joint range of motion one year after operationConclusions With our method,the position of prosthesis can be conveniently and accurately located during operation,and good postoperative effect can be achieved.TKA and UKA have definite clinical effects on single-compartment osteoarthritis,and both are the main surgical methods for the treatment of end-stage single-compartment osteoarthritis.In terms of operation time,intraoperative bleeding,postoperative joint function,and improvement of pain,UKA has obvious advantages,which is worth further promotion. |
