Analysis Of The Symptoms Relationship Of PCOS Basic Syndrome Types And Basic Study On The Construction Of Associated Syndrome Scale

Objective:Explore the symptom relationship between progressive stratification and cross-layer correlation of common basic syndrome types of polycystic ovary syndrome（PCOS）from the aspect of syndrome composition,and try to integrate the symptom relationship into t he syndrome scale,realize the two-way correlation between items and dimensions,deepen the understanding of pathogenesis,improve the accuracy of syndrome diagnosis,and provide reference ideas for the development of syndrome scale.Methods:1.PCOS literature study on the information of four diagnosis and the distribution of syndrome factors.The information of four diagnoses and syndromes of traditional Chinese medicine in the literature was selected and standardized according to symptoms,syndromes and syndrome factors.The common symptoms and syndrome factors of PCOS were analyzed and counted by frequency and frequency,and the"PCOS Clinical Information questionnaire"was established.2.Analysis of the relationship between basic syndromes and sym ptoms of PCOS.263 clinical cases were collected with"PCOS Clinical Information questionnaire".With the help of SPSSModeler18.0 software,the association rule model of basic syndrome symptoms of PCOS was constructed by Apriori algorithm.The parameters s uch as confidence,support and promotion degree were set according to the characteristics of each syndrome data,and the maximum number of antecedents was 3 to screen the symptom pairs or symptom groups with high co-occurrence probability and strong correl ation.Complete the complex network modeling of each syndrome symptom of PCOS with the help of Gephi0.9 software.The complex network characteristics of each syndrome symptom are analyzed by calculating the topological indexes such as node degree,degree di stribution,average path length,average aggregation coefficient and so on.The core symptom nodes of each syndrome are filtered by K-core method,and the importance of each syndrome core symptom node is evaluated according to the statistical characteristi cs such as intermediate center degree and eigenvector center degree,and important symptom nodes are marked.The importance of each symptom edge was evaluated by edge weight analysis,which showed the symptom pairs with high frequency of co-occurrence and strong correlation.3.Based on the symptom relationship,the items of PCOS associated syndrome scale were formed.Taking the basic syndrome of PCOS as the dimension of the scale,the core symptoms screened by complex network K-core method as the source of items,and marking the pairs or symptom groups with strong correlation between important symptoms and symptoms,the horizontal correlation item pool of PCOS was constructed.The data of 263 clinical cases collected in the previous period were replaced,an d the items were screened by correlation coefficient method and differentiation method,and finally determined that there were not only horizontal correlation of each item,but also vertical correlation between each item and the corresponding dimension of PCOS association syndrome scale items.Results:1.184 literatures were included,including 491 cases of four diagnosis information and 536 cases of syndrome factor information.121items of PCOS related symptoms and 15 items of syndrome factors were screened out,and the clinical information collection table of PCOS patients was established.2.According to the results of association rules and complex network analysis,the symptom relationship between progressive stratification and cross-layer associa tion of 7 basic syndrome types of PCOS kidney qi deficiency syndrome,kidney yang deficiency syndrome and kidney yin deficiency syndrome was determined.Among them,there are 18 core symptoms of kidney qi deficiency syndrome,7 important symptom nodes,4 c losely related symptom pairs or symptom groups,14 core symptoms of kidney yang deficiency syndrome,5 important symptom nodes,3 strongly related symptom pairs or symptom groups,10 core symptoms of kidney yin deficiency syndrome,4 important symptom nodes,and 1 symptom pair or symptom group with strong correlation.There are 14 core symptoms of liver depression syndrome,5 important symptom nodes,3 symptom pairs or symptom groups with strong correlation,12 core symptoms of spleen deficiency syndrome,4 important symptom nodes,2symptom pairs or symptom groups with strong correlation,16 core symptoms of phlegm-dampness syndrome,3 important symptom nodes and 2 symptom pairs or symptom groups with strong correlation.There were 9 core symptoms of blood stasis syndrome,1 important symptom node,and 1 symptom pair or symptom group with strong correlation.3.The symptom relationship was classified and marked to form a pool of horizontal correlation items of PCOS syndrome scale.The items of the related syndrome scale were screened by correlation coefficient method and differentiation analysis method,including 6 items of kidney qi deficiency syndrome,which were:mental fatigue ¹,burnout ¹,Shaoqi lazy speech,dizziness,tinnitus and decreased libido.The re were ⁵ types of kidney yang deficiency syndrome,which were as follows:less menstruation ²,fear of cold limbs ^{2 3},fatigue ³,mental fatigue,sore waist and legs.There were 7 types of kidney yin deficiency syndrome,which were as follows:viscous low er mass,sore waist and legs,dry mouth,hot hands and feet,yellow urine,fine pulse and red tongue.There were 6 types of liver depression syndrome,which were as follows:breast distension and pain ³,viscous substratum,irritability ^{1 2 3},sighing ²,dry mouth and dry throat ¹,depression.Six types of spleen deficiency syndrome were as follows:menstrual diarrhea ¹,fatigue,dizziness,abdominal distension after eating,loose stool ².There were 7 phlegm-dampness syndromes,which were as follows:fatigue of limbs ²,dizziness,dreaminess ²,shortness of breath,head weight,phlegm between throat and greasy fur.There are 5 types of blood stasis syndrome,which are as follows:thick menstruation,dark purple menstruation,dark lip nail,tingling in the l ower abdomen and dark tongue.Conclusion:1.PCOS kidney qi deficiency syndrome,kidney yang deficiency syndrome and liver depression syndrome screened out more clear and consistent with the pathogenesis of the core symptoms and related symptom pairs,whi ch is helpful to improve the accuracy of syndrome diagnosis,indicating that association rules and complex network analysis can be used to analyze the relationship between symptoms.2.Under the guidance of TCM theory and the principle of scale development,the screening results of all dimensions of PCOS related syndrome scale integrated with symptoms are consistent with the understanding of pathogenesis,indicating that the research idea of related syndrome scale has a certain research value.
