ESPB On PND Impairment In Elderly Patients Undergoing Laparoscopic Colon Cancer Surgery

For elderly patients undergoing radical resection of colon cancer under general anesthesia,ultrasound guided erector spinae plane block(ESPB)was performed before surgery.To evaluate its effect on postoperative pain and perioperative neurocognitive disorders(PND).Sixty elderly patients who underwent laparoscopic radical resection of colon cancer in Affiliated Hospital of Chifeng University from October 2021 to October 2022 were selected.They were randomly divided into experimental group 30 and control group 30.Informed consent and ethical review of patients and their families were obtained.Basic vital signs were taken after entry.The experimental group received ESPB,while the control group received no special treatment.Then radical operation of colon cancer was performed under intravenous aspiration combined with anesthesia.Blood pressure and heart rate,scores of the Simple Mental State Assessment Scale(MMSE),and venous blood were recorded at T0(12h before surgery),T1(24h after surgery),and T2(72h after surgery),respectively,and the level of neuron-specific enolase NSE was detected.After the operation,anesthesia time,operation time,fluid intake,urine volume and blood loss were recorded.VAS scale and P nausea score were recorded at T1(24h after surgery).The results showed as follows: compared with the control group,MMSE scores of the experimental group were higher than those of the control group at 24 h and 72 h after surgery(P < 0.05).The number of patients with PND in the experimental group was less than that in the control group 24 h after operation(P < 0.05).The level of NSE in the experimental group was lower than that in the control group at 24 h and 72 h after surgery(P < 0.05),and the level of NSE in the experimental group was decreased more significantly(P <0.05).Patients with moderate or above pain in the experimental group were less than those in the control group 24 h after surgery(P < 0.05).In elderly patients undergoing radical resection of colon cancer,preoperative ESPB can achieve better analgesic effect,which can reduce postoperative pain and reduce the occurrence of PND.
