BackgroundPolycystic Ovary Syndrome(PCOS)is a lifelong disease that commonly occurs in women of reproductive age and involves multisystemic reproductive endocrine metabolic dysfunction,affecting the patient’s immediate quality of life and future health status.The current treatment of patients is mostly focused on alleviating clinical symptoms of Kaohsiung,regulating menstrual cycle and improving reproductive function,but the long-term management is inadequate and there is a lack of simple indicators to guide the patients.Insulin resistance plays an important role in the pathogenesis and progression of PCOS,and we speculate that its related indexes may have some clinical application for the diagnosis and long-term management of PCOS.There are few studies on the application of insulin resistance-related indicators to the diagnosis and long-term management of PCOS.There is an inextricable relationship between insulin resistance and hyperandrogenism,and hyperandrogenism leads to an increase in the degree of insulin resistance in the body.We speculate that insulin resistance-related indicators may be relevant indicators to reflect biochemical hyperandrogenism in PCOS,and there are few studies on the relationship between insulin resistance levels and hyperandrogenism in PCOS.ObjectiveTo investigate the correlation between FINS,HOMA-IR and biochemical hyperandrogenism levels in PCOS;to analyze the compliance rate of FINS,HOMA-IR and FAI in diagnosing biochemical hyperandrogenemia in PCOS;to discuss the significance of FINS and HOMA-IR as relevant indicators reflecting biochemical hyperandrogenism levels in PCOS;to explore the significance of monitoring FINS and HOMA-IR in the long-term management of PCOS patients.The significance of monitoring FINS and HOMA-IR in the long-term management of PCOS patients is discussed.To provide new ideas for the diagnosis and long-term management of PCOS patients.MethodsOne hundred and one patients with PCOS were selected as the observation group and 73 healthy women were selected as the control group,and clinical data,relevant serological indexes and imaging indexes were collected from all subjects.The non-parametric Mann-Whitney U test was used to analyze the differences of serological indexes between the observation group and the control group.Pearson linear correlation and multiple stepwise regression were used to study the correlation between FINS,HOMA-IR and each serological index,and the ROC curve was used to select the diagnostic thresholds of FINS and HOMA-IR for biochemical hyperandrogenemia,respectively.The McNemar test was used to evaluate the consistency of FINS,HOMA-IR and FAI in the diagnosis of biochemical hyperandrogenemia.ResultMann-Whitney U test analyzed the differences between serological indexes in the observation group and the control group,and the levels of BMI,TG,LDL,FINS,T,FAI,and HOMA-IR were significantly higher(P<0.01)and the levels of SHBG and HDL were lower(P<0.01)in women in the observation group than in the control group;Pearson linear correlation analysis showed that patients in the observation group HOMA-IR was positively correlated with FAI,BMI and TG(P<0.001)and negatively correlated with SHGB,LH and HDL(P<0.001),while FINS was positively correlated with FAI,BMI and FBG(P<0.001)and negatively correlated with SHGB,LH and HDL(P<0.001)in the observation group;multiple stepwise regression analysis found that FAI was an independent risk factor affecting the levels of FINS and HOMA-IR in the observation group;the results of McNemar test and Kappa consistency test suggested that the differences between FINS,HOMA-IR and FAI diagnosed biochemical hyperandrogenemia were not statistically significant.ConclusionFAI is an independent risk factor affecting HOMA-IR and FINS levels in PCOS patients;FINS and HOMA-IR can be relevant indicators reflecting the degree of biochemical hyperandrogenemia in PCOS;HOMA-IR and FINS are better indicators for diagnosing biochemical hyperandrogenemia in PCOS patients as FAI;regular monitoring of FINS and HOMA-IR has certain clinical guidance significance in the long-term management of PCOS.
