Objective: To observe the clinical efficacy and safety of modified Tianma-Gouteng decocti-on in the treatment of residual symptoms after repositioning of benign paroxysmal positional vertigo with hyperthyroidism of liver-Yang.Methods: The cases selected in this study were all from 80 patients with residual symptoms of benign paroxysmal positional vertigo of hyperthyroidism of liver-Yang diagnosed in the outpatient and ward of the Department of Encephalopathy of Weifang Hospital of Traditional Chinese Medicine from November 2021 to November 2022.The patients were randomly divided into the treatment group and the control group using SPSS26.0 software,with 40 cases in each group.Betahistine mesylate was given orally in the control group,and Tianma-Gouteng decoction was given orally in the treatment group.The course of treatment in both groups was 14 days.The general condition of the patients,the duration of residual symptoms and the recurrence of BPPV within 3 months were recorded.The changes of vertigo handicap inventory(DHI),visual analogue scale(VAS)and traditional Chinese Medicine syndrome Evaluation scale(TCMSS)scores before treatment,1 week after treatment and 2 weeks after treatment were compared between the two groups.The therapeutic effects of the two groups were evaluated.Finally,SPSS26.0 software was used for statistical analysis.Results: A total of 80 patients were included in this study,of which 4 patients dropped out or were eliminated during the experiment,and there were 2 patients in the control group and the treatment group,respectively.Finally,a total of 76 cases completed the experiment and scored,38 cases in each group.All data were entered and analyzed by SPSS26.0software.After statistical analysis,there was no significant difference in general conditions between the two groups(P>0.05),which was comparable.Results:(1)The total effective rate of the treatment group was higher than that of the control group(78.95% vs 60.53%),and the difference was significant(P<0.05).(2)The scores of the two groups were lower than those before treatment;After 2 weeks of treatment,the improvement of DHI total score,DHI-P,E,F3 subscale scores,VAS score and TCMSS score in the treatment group were better than those in the control group,and the differences were statistically significant(P<0.05).The clinical efficacy of modified Tianma-Guteng decoction in the treatment of residual symptoms of benign paroxysmal positional vertigo with hyperthyroidism of liver-Yang was better than that of betamethylate(3).The recurrence rate of the control group was higher than that of the treatment group(3.95%>2.63%),but the difference was not statistically significant(P<0.05).(4)There was no significant difference in the duration of residual symptoms between the two groups after successful repositioning of benign paroxysmal positional vertigo(P>0.05).(5)There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05),and no serious adverse reactions occurred.(6)There was no significant difference in the evaluation of safety indexes between the two groups before and after treatment(P>0.05),which proved that both drugs were safe.Conclusion: Modified Tianma-Hooteng decoction in the treatment of residual symptoms after repositioning of benign paroxysmal positional vertigo with hyperthyroidism of liver-Yang can effectively reduce DHI,VAS and TCMSS scores.There were no serious adverse events during the treatment and follow-up.The clinical efficacy of modified Tianma-Gouteng decoction in the treatment of residual symptoms of benign paroxysmal positional vertigo with hyperthyroidism of liver-Yang is better than that of betastone mesylate,and the modified Tianma-Gouteng decoction is safe and effective.Further clinical trials and clinical promotion can be considered. |
