Analysis Of The Risk Factors And TCM Syndrome Characteristics Of Hypertension With Hyperuricemia In Young And Middle-aged People

Objective: To explore the related factors of young and middle-aged hypertension with hyperuricemia(HUA),and to establish a predictive model to determine the risk factors of the disease.At the same time,the distribution characteristics of TCM syndromes of the disease and the distribution of risk factors among different syndromes were explored.In order to provide reference for further TCM and western medicine prevention and treatment of hypertension with hyperuricemia in young adults.Methods: Using the electronic medical record management system of the Affiliated Hospital of Shandong University of traditional Chinese medicine,we selected the young and middle-aged patients with hypertension and complicated with Hua who were in accordance with the inclusion and exclusion criteria,and established the database by using EXCEL2021,s PSS25.0 was used for retrospective analysis.Results: 1.A total of 492 cases were included in this study,including255 cases of young and middle-aged hypertension with HUA and 237 cases of hypertension without HUA.2.Age,sex,Body Mass Index(BMI),smoking history,alcohol consumption,type 2 diabetes mellitus,hyperlipidemia,carotid intima-media thickness,triglyceride,total cholesterol and highdensity lipoprotein were compared between the HUA group and the non-HUA group,the difference was statistically significant(P<0.05).In the HUA Group,male,smoking,drinking,type 2 diabetes mellitus,hyperlipidemia(HCY),carotid intima-media thickness,triglyceride and total cholesterol were significantly higher than those in the non-HUA group,BMI in HUA Group was higher than that in non-HUA group.Age and high-density lipoprotein in HUA group were lower than that in non-HUA group.The course of hypertension,family history of hypertension,hypertension classification and risk stratification,complicated with coronary heart disease,complicated with cerebral infarction,complicated with thyroid disease,cervical arteriosclerosis,fasting blood glucose,Low-density lipoprotein and Hcy were compared between the two groups,no significant difference was found(P>0.05).3.The factor of P<0.05 in the result of single factor group comparison,age,sex,BMI,smoking,alcohol consumption,type 2 diabetes mellitus,hyperlipidemia,carotid intima-media thickness,triglyceride,total cholesterol,high-density lipoprotein,the above-mentioned indexes were put into binary logistic regression equation,results male,18-44 years old,overweight and obesity,alcohol consumption and total cholesterol were the independent risk factors of HUA(OR>1,P<0.05).4.The distribution of TCM syndrome types of hypertension complicated with HUA in young and middle-aged people: 92 cases(36.1%)with excessive phlegm-dampness syndrome>65 cases(25.5%)with excessive liver-fire syndrome > 51 cases(20%)with yin-deficiency and yang-hyperactivity syndrome>47 cases(18.4%)with yin-yang deficiency syndrome in HUA Group;In non-HUA group,81 cases(34.2%)were hyperactivity of liver-fire,56cases(23.6%)were hyperactivity of phlegm-dampness,51 cases(21.5%)were deficiency of both yin and yang,and 49 cases(20.7%)were hyperactivity of yin and yang,there was a significant difference in the distribution of syndrome types between the two groups(χ~2=10.47,P<0.05).5.The above independent risk factors and uric acid(UA)values were compared between middle-aged and young people with hypertension and HUA syndrome,and it was found that there were statistical differences in UA level and BMI between different types(P < 0.05),while there were no significant differences in age,sex,alcohol consumption and total cholesterol between different syndrome types(P>0.05).Conclusion: 1.HUA was associated with gender,age,BMI,diabetes,hyperlipidemia,smoking,alcohol consumption,carotid intima-media thickness,triglyceride,total cholesterol and high-density lipoprotein.2.Male,18-44 years old,overweight and obesity,drinking history,total cholesterol were the risk factors of HUA in young and middle-aged hypertensive patients.3.The distribution of TCM syndrome types of hypertension in young and middle-aged patients presented hyperactivity of liver-fire>hyperactivity of phlegm-dampness>deficiency of both yin and yang>Hyperactivity of Yang due to Yin deficiency,the distribution of TCM syndrome types of hypertension complicated with HUA in young and middleaged patients presented the syndrome of excessive phlegm-dampness>the syndrome of excessive liver-fire>the syndrome of Yin deficiency and Yang hyperactivity>the syndrome of both yin and yang deficiency.It is suggested that the distribution of TCM syndrome of hypertension patients in young and middle-aged patients with HUA has changed.4.The UA level in the syndrome of excessive accumulation of phlegm-dampness was the highest in the middle-aged and young patients with hypertension complicated with HUA.The UA level has certain reference significance for the differentiation of excessive accumulation of phlegm-dampness in HUA.5.The type of TCM syndrome of hypertension in young and middle-aged patients with overweight or obesity is mainly phlegm-dampness accumulation type.BMI has some reference value for the diagnosis of phlegm-dampness syndrome in young and middle-aged hypertensive patients with HUA.
