Objective: In this study,colorectal cancer screening was carried out in four representative urban and rural communities in Linyi,and the screening data and results were analysed to provide reference for exploring suitable screening patterns for colorectal cancer in the city,and at the same time,the distribution pattern of TCM constitution types in the high-risk group of colorectal cancer was investigated in conjunction with TCM constitution identification,so as to provide guidance for "individualized" Chinese medicine prevention for the high-risk group of colorectal cancer.The study will provide guidance for the prevention of colorectal cancer among people at high risk of colorectal cancer by tailoring Chinese medicine to their individual needs.Methods: In this study,a cross-sectional stratified random sampling method was used to select Lanshan District and Junan County in Linyi City according to the level of economic development and geographical distribution,and two representative urban and rural communities were randomly selected in each of Lanshan District and Junan County,making a total of four communities.In each community,120 residents aged 45-74 years who met the inclusion criteria were selected,and the residents were initially screened for colorectal cancer using the "Colorectal Cancer Screening High Risk Factor Quantification Scale" in the "Expert Consensus Opinion on Early Colorectal Cancer Screening Process in China(2019,Shanghai)",and then the high-risk group for colorectal cancer was The Syndecan 2 gene methylation stool test will be performed on those with positive results,and diagnostic colonoscopy will be performed on those found to have pre-cancerous lesions such as polyps and colorectal cancer,as well as pathological diagnosis and follow-up.At the same time,the residents were identified according to the Chinese Medicine Physique Classification Standard(ZYYXH/T157-2009).All the above were analysed using Microsoft Excel to create a database and SPSS26.0 statistical software.Results: There were 430 of 480 cases of urban and rural population who voluntarily participated in colorectal cancer screening,all of whom received questionnaires for people at high risk of colorectal cancer,with a compliance rate of 89.6% for the initial screening and a positive rate of 36.5% for the questionnaires.98.1% of the residents were compliant with the initial screening with the SDC2 test kits,and the overall sampling efficiency of the kits was 94.2%,with a positive rate of 20.0% for the kits.The number of positive individuals willing to undergo colonoscopy was 82.8%,and the detection rate of colorectal cancer and precancerous lesions in colonoscopy was 58.3%.The location of polyps was predominantly colon,accounting for67.8% of the cases.The majority of polyps were flat,accounting for 58.1%of the total,and the majority of polyps were adenomatous,accounting for 81.3%of the total.The distribution of TCM constitution types in the colorectal cancer high-risk group is dominated by Damp-Heat and Yang-Deficiency types,accounting for 48.4% and 20.4% respectively.By chi-square test,the differences between age,community and colorectal cancer primary screening compliance,and the differences in questionnaire positivity rates were statistically significant(P<0.05),and the differences between community and SDC2 gene methylation kit sampling efficiency were statistically significant(P<0.05).By binary logistic regression analysis,there was a strong association between TCM body type and colorectal cancer risk group,with Damp-Heat(OR=33.657)and Yang-Deficiency(OR=19.84)being risk factors for the occurrence of colorectal cancer in the high-risk group(P<0.001).Conclusion:1.The community-led screening model of "Colorectal Cancer Screening Risk Factor Quantification Form + SDC2 Gene Methylation Stool Test + Colonoscopy",adopted by the designated hospitals,is conducive to increasing the participation rate of colorectal cancer screening.2.The "SDC2 methylation stool test combined with colonoscopy" is beneficial in improving the acceptance rate of colonoscopy and the accuracy of detection of colorectal cancer and precancerous lesions.3.The TCM constitution of people at high risk of colorectal cancer is mainly biased,with the balanced constitution being a strong protective factor,and the dampness-heat constitution being a risk factor for the development of colorectal cancer in people at high risk of colorectal cancer.This theory can provide guidance for the future prevention of colorectal cancer in high-risk groups by tailoring Chinese medicine to the individual.
