Objective:To investigate the value of tumor associated material(TAM)in the screening of patients with different malignant tumors,and to provide help for the early diagnosis of patients with malignant tumors.In addition,the expression of TAM in patients with malignant tumors before and after chemotherapy was analyzed,so as to conclude the relevant value of TAM detection in clinical diagnosis,treatment and prognosis of patients with different tumors.Methods:Selecting 412 malignant tumor patients(113 cases in colorectal cancer group,130 cases in gastric cancer group,92 cases in primary liver cancer group,and 77 cases in breast cancer group)who were diagnosed and treated in the 967 Hospital of the Chinese People’s Liberation Army Joint Logistic Support Force from January 2017 to December2021 as the observation group,and 83 healthy physical examination individuals as the control group.In the meantime,50 patients who received chemotherapy were selected from each malignant tumor group in the observation group and divided into four groups:colorectal cancer chemotherapy group,liver cancer chemotherapy group,gastric cancer chemotherapy group and breast cancer chemotherapy group.Peripheral blood samples of all the selected patients and peripheral blood samples of patients in each chemotherapy group before and after chemotherapy were collected.and the TAM level was quantitatively detected by colorimetric method.Statistical software SPSS 26.0 was used to analyze the differences in serum TAM levels among the groups,respectively using two independent sample t-test to analyze the difference between groups,and using receiver operating characteristic(ROC)curve to analyze the diagnostic efficacy of TAM for malignant tumor.Paired sample T test was used to analyze the trend of TAM before and after chemotherapy in patients with malignant tumors.Results:The specificity of tumor associated material(TAM)level in healthy subjects was75.90%.The sensitivity of 412 patients with malignant tumor was 67.48%,and the sensitivity of each tumor group was 79.65% for colorectal cancer,67.39% for liver cancer,66.92% for gastric cancer and 50.65% for breast cancer.Using two independent sample t-test,the results showed that the average TAM level of colorectal cancer group was higher than that of the healthy group,p<0.05,and the difference was statistically significant;the average TAM level of liver cancer group was higher than that of the healthy group,p<0.05,and the difference was statistically significant;the average TAM level of gastric cancer group was higher than that of the healthy group,p<0.05,and the difference was statistically significant;the average TAM level of breast cancer group was higher than that of the healthy group,p<0.05,and the difference was statistically significant.The summarized ROC(receiver operating characteristic)Curve was used to analyze the diagnostic efficacy of TAM on malignant tumors.The results showed that The value of AUC(Area under curve)under ROC curve for the diagnosis of colorectal cancer at TAM level was 0.762(P<0.05),indicating that serum TAM had statistical significance in the diagnosis of colorectal cancer.The AUC value of TAM concentration in the diagnosis of liver cancer was 0.743,P<0.05,and serum TAM was statistically significant in the diagnosis of liver cancer.the AUC value of TAM level for diagnosing gastric cancer was 0.751,P<0.05,and the serum TAM had statistical significance for diagnosing gastric cancer;the AUC value of TAM level for diagnosing breast cancer was 0.677,P<0.05,indicating that serum TAM had statistical significance in the diagnosis of breast cancer.Meanwhile,AUC in the diagnosis of colorectal cancer at TAM level was larger than that in other groups,indicating that the diagnostic efficacy of TAM for colorectal cancer was better than that in other groups.Paired sample T test was performed for TAM levels before and after chemotherapy in colorectal cancer chemotherapy group,liver cancer chemotherapy group,gastric cancer chemotherapy group and breast cancer chemotherapy group.The results showed that the average TAM level after chemotherapy in colorectal cancer chemotherapy group was significantly lower than that before chemotherapy,p<0.05,and the difference was statistically significant;the average TAM level after chemotherapy in liver cancer chemotherapy group was slightly lower than that before chemotherapy,p>0.05,and the difference was not statistically significant;the average TAM level after chemotherapy in gastric cancer chemotherapy group was slightly lower than that before chemotherapy,p>0.05,and the difference was not statistically significant;the average TAM level after chemotherapy in breast cancer chemotherapy group was slightly lower than that before chemotherapy,p>0.05,and the difference was not statistically significant.Conclusions:Based on the analysis of TAM data of 83 healthy physical examination individuals and 412 malignant tumor patients in our hospital,the preliminary clinical study of TAM detection application in malignant tumor diagnosis and the trend of serum TAM concentration before and after chemotherapy in malignant tumor patients,we can draw the following conclusions:1.The serum concentration of tumor associated material(TAM)in patients with malignant tumors(colorectal cancer,liver cancer,stomach cancer,breast cancer)is higher than that in healthy people,and TAM detection has higher sensitivity and specificity for the diagnosis of malignant tumors,which is of relatively important value for the early diagnosis and screening of malignant tumors.2.Serum TAM test can be used to diagnose colorectal cancer,liver cancer,stomach cancer and breast cancer,and compared with liver cancer,stomach cancer and breast cancer,TAM has the highest sensitivity and the best diagnostic efficacy in colorectal cancer.3.Chemotherapy can significantly reduce the serum TAM level of patients with colorectal cancer,which is of certain value for evaluating the efficacy of chemotherapy and prognosis of patients with colorectal cancer.
