Purpose:From two aspects of internal and external use of traditional Chinese medicine,traditional Chinese medicine used in the clinical application of upper limb dysfunction after stroke was summarized and analyzed by using traditional Chinese medicine inheritance computing platform,in order to obtain effective traditional Chinese medicine for the treatment of such diseases.At the same time,the network pharmacology technology was used to predict and analyze the obtained core herbal medicine combination,in order to obtain its mechanism of action in the treatment of upper limb dysfunction after stroke.From clinical to basic research level,it provides scientific basis for TCM treatment of upper limb dysfunction after stroke.Methods:1.Data collection: Data were collected from CNKI,Wanfang and Pub Med databases since the establishment of the database.Chinese and English literature on the internal and external use of traditional Chinese medicine for upper limb dysfunction after stroke was consulted.2.The traditional Chinese medicine inheritance computing platform was used to analyze the data related to internal and external use of traditional Chinese medicine in the medication pattern of upper limb dysfunction after stroke: the frequency of occurrence of drugs was analyzed and summarized to obtain drugs with high frequency.To summarize the four qi,five taste,meridian conversion and efficacy of drugs,and get a wide range of clinical drug use rules.K-means algorithm was used to obtain the cluster analysis chart,which intuitively showed the related classification and core drug composition.Association rules were used to analyze the prescription,and different core drug combinations were obtained with different levels of support and confidence.3.Network pharmacological technology was used to analyze the mechanism of action of the core oral Chinese medicine combination in the treatment of upper limb dysfunction after stroke: the effective ingredients and potential targets of the core oral Chinese medicine combination were obtained by using TCMSP and Uniprot database;Potential targets of "Upper limb dysfunction" and "Stroke" were obtained by using Gene Cards and OMIM databases;Venny2.1.0 platform was used to draw the Venn diagram,and the intersection genes between the core herbal combination and the disease were obtained.STRING database was used to draw protein interaction network diagram,and R software was used to get protein target frequency diagram.Cytoscape3.8.2 was used to obtain the network diagram of "drug-composition-target" and "drug-target-disease".Network Analyze is used to analyze network topology parameters and screen active components.Using R software org.Hs.eg.db,Bioc Manager,Cluster Profiler,and Enrichplot,GO enrichment and KEGG enrichment are analyzed.Results:1.Medication rules of internal administration of Chinese medicine for upper limb dysfunction after stroke: A total of 90 prescriptions and 138 TCM were collected and screened.According to systematic analysis,the occurrence frequency of the four-gas module of drugs was warm drugs,followed by cold and peaceful drugs.The five tastes of medicine appeared most frequently as bitter,sweet and sweet.The highest frequency of drug channel conversion was liver channel,followed by spleen,heart,lung and kidney channel.Drug efficacy appeared more frequently for supplementing deficiency,promoting blood circulation and removing blood stasis,etc.The ranking drugs with frequency greater than 50 were ligusticum Chuanxiong,Angelica sinensis,Astragalus membranaceus,Dilong and safflower,respectively.According to the network topology,the core drug combinations were ligusticum Chuanxiong,angelica sinensis,safflower,peach kernel,Dilong,red peony root,astragalus,cassia branch.2.Medication rules of external use of traditional Chinese medicine in the treatment of upper limb dysfunction after stroke: According to the principle of external use of traditional Chinese medicine,the analysis is divided into two parts,including direct skin contact drugs and indirect skin contact drugs.A total of 69 prescriptions and148 traditional Chinese medicines were collected and screened for direct contact with skin.Indirect contact skin category,a total of 49 prescriptions,129 traditional Chinese medicine.In the four Qi analysis of the topical TCM drugs with direct skin contact and indirect skin contact in the treatment of upper limb dysfunction after stroke,the frequency of occurrence was most warm drugs,and the frequency of five drugs was most occinative drugs and bitter drugs,respectively.Hepatic channel was the most frequent drug normalization.The efficacy frequency of direct contact with skin was most for promoting blood circulation and removing blood stasis and rheumatism,while the efficacy frequency of indirect contact with skin was most for promoting blood circulation and removing blood stasis.The top 10 Chinese herbs with direct contact frequency were safflower,Angelica sinensis,cassia twig,Shenjin grass,Ligusticum Chuanxiong,mulberry twig,Herbaceous grass,radix aconitum,Radix aconitum and frankincense.The top 10 medicines with indirect contact with the skin were safflower,Ligusticum Chuanxiong,Angelica sinensis,Shenjin grass,spatholobi spatholobi,Turtlenia spatholobi,cassia twig,frankincense,myrrh and Qianghua.According to the network topology,it can be concluded that the core drug combinations that directly contact the skin are astragalus membranaceus,angelica sinensis,ligusticum Chuanxiong,safflower,mulberry branch,cassia branch and Sinjin grass.The core drug combinations of indirect contact with the skin are Gentiana Gentiana,Sinjin grass,safflower,spatholobi,Ligusticum ligusticum,Angelica sinensis,frankincense,and Dioscorea.3.Mechanism of action for the treatment of upper limb dysfunction after stroke by the combination of core oral Chinese medicine: 64 effective components and 214 potential targets of the combination of core oral Chinese medicine;288 potential disease targets;There were 25 intersection targets of the core internal administration of TCM combination and disease.High frequency core protein targets were AKT1,ALB,TNF,IL6,etc.The main active components of the core herbal combination were quercetin,β-sitosterol,luteolin,baicalin,β-carotene,etc.38 items were obtained by GO enrichment analysis.More concentrated are growth factor receptor binding,protease binding,serine-type endopeptidase activity,etc.A total of 58 items were obtained by KEGG enrichment analysis.The most concentrated are AGE-RAGE signaling pathway in diabetic complications,Fluid shear stress and atherosclerosis atherosclerosis,Complement and coagulation cascades,etc.Conclusion:1.Through the calculation platform of traditional Chinese medicine inheritance,the analysis of the treatment of upper limb dysfunction after stroke by taking traditional Chinese medicine internally shows that the most frequently used drugs are warm and bitter drugs and return to the liver meridian.It can be inferred that the sequelae of stroke are mainly Qi deficiency and blood stasis syndrome,and the most frequently used drugs are ligusticum ligusticum,Angelica sinensis,astragalus membranaceus,Earth long and safflower.The core drug combination was based on Buyang Huanwu Decoction and Astragalus Guizhi Wuwu decoction(Ligusticum Chuanxiong,angelica sinensis,Honghua,peach kernel,Dilong,red peony root,Astragalus and cassia Guizhi).2.Analysis of the treatment of upper limb dysfunction after stroke by external administration of traditional Chinese medicine through the calculation platform of traditional Chinese medicine inheritance showed that the direct contact with the skin mainly included warm and occinative drugs,liver channels,and rheumatic drugs.The drugs with high frequency were safflower,angelica sinensis,cassia twigs,Shenjincao and ligusticum Chuanxiong,etc.The core drug composition was added or subdivided on the basis of Buyang Huanwu Decoction as the main formula.The indirect contact with the skin is mostly warm and bitter drugs,return to the liver meridian,promote blood circulation and remove blood stasis more drugs,the high frequency of drugs are safflower,ligusticum Chuanxiong,angelica,Shenjin grass,caulis spatholobi,etc.Core drug composition was added or subtracted on the basis of Taohong Siwu decoction as the main formula.Since most of the data for external use of traditional Chinese medicine come from self-made prescriptions in the hospital,there are problems such as few similarities and incomplete data analysis.In future studies,data will be collected and analyzed continuously,hoping to be helpful for clinical application.3.The analysis of the treatment of upper limb dysfunction after stroke by the combination of core herbal medicine through the network pharmacological technology.The active ingredients of the combination of core herbal medicine in the treatment of upper limb dysfunction after stroke may be quercetin,β-sitosterol,luteolin,baicalin,β-carotene,etc.,which may act on potential targets such as AKT1,ALB,TNF and IL6.The AGE-RAGE signaling pathway,fluid shear stress and atherosclerosis,complement and coagulation cascade in diabetic complications are regulated to improve patients’ clinical symptoms.
