Clinical Efficacy Of 3M Filtek^TM Bulk Fill Light-cure Composite Resin Combined With EVE DIACOMP PLUS TWIST Polishing Wheel For Caries-filled Restorations

Objective:In the form of a case report,3M Filtek^TM Bulk Fill light-cure composite resin and EVE DIACOMP PLUS TWIST polishing wheel were applied to posterior dental caries filling restorative treatment,compared to clinical conventional resin filling.The clinical condition of resin restorations in the four groups after filling treatment at 6 and 12 months were compared through follow-up observation,to explore the clinical efficacy of 3M Filtek^TM Bulk Fill light-cure composite resin and EVE DIACOMP PLUS TWIST polishing wheel applied to posterior dental caries filling restorative treatment.Methods: According to the order of patient visits,120 affected teeth（all permanent teeth）that met the inclusion criteria were randomly divided into 4 groups of30 teeth each.group ZS: Z350 XT + SILICON One Gloss;group ZE: Z350 XT + EVE TWIST;group BS: Bulk Fill + SILICON One Gloss;group BE.Bulk Fill + EVE TWIST.All operations were performed by the same physician,and the same assistant cooperated and completed the timing during the treatment.Patients gave informed consent to this study.X-ray and intraoral images were taken before the treatment procedure.Oral hygiene instruction（OHI）and cleaning of the operative area procedures were explained to the patient.Under natural light,use the visual colorimetric method to select the appropriately colored resin material according to the color of the wet state of the affected and adjacent teeth.Use red and blue articulating paper to record the central and lateral occlusal contact points.Rubber dam were used to isolate the wet affected teeth.Use high-speed diamond round bur to remove a large area of carious dentin tissue and inorganic enamel,spoon excavator and low-speed round bur to remove deep carious tissue.Probe to check for the dentin hardness,and the softened dentin has been completely removed.The edges of the cavity are trimmed with standard grit diamond bur so that the edges are rounded and uninterrupted,and the profile line is rounded and gently curved;the cavity edges are polished with a very fine grit yellow bond polishing diamond bur,and the cavity edges of the occlusal surface are polished with the diamond bur parallel to the long axis of the tooth,without preparing the short bevel of the cavity edges;the cavities of the buccal surface of the mandibular molar and the palatal surface of the maxillary molar are prepared with 45° bevel of the cavity edges 0.5-1mm wide.The cavity was cleaned and blown dry.Use of Thera Cal LC caps for areas close to the pulp and light cured for 20 s.The cavity margin enamel was acid etched with 37%phosphate gel for 30 s,rinsed under pressure,and the cavity was blown dry again,the enamel at the cavity margin was chalky white.3M Single Bond^TM Universal adhensive was applied uniformly for 20 s,lightly blown for 5 s to give a non-moving glossy film on the surface,and light cured for 10 s.The bottom of the cavity was based with Magister LV2 fluid resin and light cured for 10 s.The depth of the cavity after basing was measured by a periodontal probe.The timing was started after the cavity depth measurement was recorded and ended after the resin filling and curing was completed.Group ZS,group ZE: Z350 XT layered filling,each layer ≤2mm;group BS,group BE:Bulk Fill bulk-filling.Remove the rubber dam,check the occlusal contact points,and modify and polish.Group ZS and group BS: SILICON One Gloss polishing for 30 s;group ZE and group BE: EVE TWIST polishing wheel polishing for 30 s.Intraoral photographs were taken immediately after treatment.Patients were informed of the next follow-up appointment,precautions to be taken after the filling treatment and what might happen.Results: 1.Six months after filling restorations,compared various clinical efficacy of the four groups of restorations,and there was no statistical difference（P > 0.05）.However,in the three observed indexes of marginal adaptation,marginal staining,and surface morphology,the group ZE had a higher number of A grades and no C grades evaluated than the group ZS and the group BE than the group BS;there was no significant difference between the ZS and BS groups and between the ZE and BE groups.2.Twelve months after filling restorations,there was no statistical difference（P >0.05）when comparing the various clinical outcomes of the four groups of restorations.However,in the three observed indexes of marginal adaptation,marginal staining,and surface morphology,the group ZE had a higher number of A grades and no C grades evaluated than the group ZS and the group BE than the group BS;there was no significant difference between the ZS and BS groups and between the ZE and BE groups.3.The operation time was shorter in BS and BE groups than in ZS and ZE groups,which was statistically different（P < 0.05）;between ZS and ZE groups and between BS and BE groups,the operation time was not statistically different（P > 0.05）.Conclusions: 1.The clinical efficacy of 3M Filtek^TM Z350 XT and 3M Filtek^TMBulk Fill restoration of deep caries Class Ⅰ cavities in posterior teeth met the requirements for clinical application at six and twelve months after restoration filling,with no statistical difference.2.Six and twelve months after the filling restoration,the EVE TWIST polishing wheel had better clinical results than the SILICON One Gloss in three aspects: marginal adaptation,marginal staining,and surface morphology,but there was no statistical difference.3.Under the conditions of this study,the average 3M Filtek^TM Bulk Fill operation time is statistically significantly shorter than 3M Filtek ^TM Z350 XT.This experiment suggests that the effect of 3M Filtek^TM Bulk Fill to repair deep caries Class I cavities can meet the requirements of clinical application,while improving the efficiency of clinical operation,shortening the chairside time,and improving the comfort of patient visits.Combined with the EVE TWIST polishing wheel for polishing,it can reduce the occurrence of edge gaps to a certain extent,reduce marginal staining,and maintain the surface morphology for clinical application to provide reference.
