Background: As an inflammatory skin disease characterized by exudation tendency,eczema is mainly divided into three stages: acute,subacute and chronic.The course of chronic eczema can last from months to decades,and the itching is severe,which has seriously affected the quality of life and mental health of patients.Although traditional topical glucocorticoids can control symptoms,they are easy to rebound after withdrawal,and have certain side effects.It is not recommended to use large amounts of glucocorticoids in chronic patients for a long time.A large number of experimental results show that aromatic hydrocarbon receptor agonists can regulate the formation,growth and differentiation of epidermal keratinocytes,and inhibit the inflammatory response caused by IL-4,TNF-α and other cytokines,and have certain therapeutic effect on eczema,atopic dermatitis and other skin inflammatory diseases.In the future,Benvermod may be more widely used in the field of inflammatory diseases such as eczema and atopic dermatiti.Objective: By observing the clinical effect of Benvermod cream on patients with chronic eczema and its effect on mice models of eczema,the clinical efficacy and possible mechanism of Benvermod cream on eczema were investigated.Methods: A total of 86 patients diagnosed with chronic eczema in the Department of Dermatology of Subei People’s Hospital from November of 2020 to November of 2023 were selected and randomly divided into experimental group(topical Benvimod cream)and control group(topical hydrocortisone butyrate cream),with 43 patients in each group.EASI score of eczema area and severity index,VAS score of skin pruritus index,DLQI score of dermatosis life quality index,efficacy grade and effective rate were recorded before medication,1 week after medication and 2 weeks after the treatment,respectively.Forty male BALB/C mice aged 6 to 8 weeks were randomly divided into experimental group,control group,model group and blank group,with 10 mice in each group.The other three groups of experimental mice except the blank group were induced into eczema models by 2,4-dinitrochlorobenzene(DNCB solid dissolved in acetone olive oil solution).After successful modeling,Benvimod cream was applied externally in the experimental group,and hydrocortisone butyrate cream was applied externally in the control group,once a day;The model group and blank group were not treated for 7 days.The vital signs,body weight and EASI score of the mice were observed and recorded.After 7 days of administration,the mice were sacrificed,and the back skin lesions were stained by HE for pathological examination;Serum was extracted from the eyeballs of mice,and the levels of IL-4 and TNF-αin serum of mice in each group were detected by Elisa.SPSS software 22.0 was used for statistical analysis of the data.Results: 1.Comparison of clinical efficacy of Benvimod cream for patients with chronic eczema: 41 patients in the experimental group and 40 patients in the control group completed the trial,noserious adverse reactions occurred.The EASI evaluation,VAS evaluation and DLQI evaluation in the experimental group after treatment showed a significant tendency to decrease(p<0.001);However,in week 1 and week 2,the scores of EASI and DLQI in the experimental group were higher than that in the control group,and the difference was statistically significant(p<0.05);The clinical efficacy grade and effective rate of the experimental group were also lower than that of the control group,and the difference was statistically significant(p<0.05).2.Effect of Benvimod cream on DNCB-induced eczema model in mice: After application,the EASI scores in the experimental group showed a significant downward trend,and the difference was statistically significant(p<0.001);The EASI score of the experimental group decreased less than that of the control group,and the difference was statistically significant(p<0.05);The epidermal edema and dermal inflammatory cells in HE pathological sections of back lesions of mice in the experimental group was similar to that in the control group,but mild than that in the model group;The serum of IL-4 and TNF-α of experimental group and control group were significantly decreased compared with model group,and the difference was statistically significant(p<0.05).Conclusion: Benvimod cream can significantly improve the skin lesions of chronic eczema,improve the quality of life of patients,reduce the skin inflammation of mouse eczema model induced by DNCB,and reduce serum IL-4 and TNF-α in mice.However,Benvimod cream is less effective than hydrocortisone butyrate cream.
