Research On The Recipe And Syndrome Of Guizhi Plus Houpu Xingzi Decoction Used By Professor Li Weiwei In Pediatric Pulmonary Diseases Based On Data Mining

Objective:Through data mining technology,the medical case of applying Guizhi plus Houpu Xingzi decoction in the outpatient department of Professor Li Weiwei was excavated and analyzed,so as to explore the prescription and syndrome rule of Guizhi plus Houpu Xingzi decoction in pediatric pulmonary diseases,and summarize the prescription and syndrome rule and experience of Professor Li Weiwei in applying this prescription in pediatric pulmonary diseases.To better carry on the inheritance and innovation of TCM classics,and provide reference for clinical diagnosis and treatment.Methods:From December 2018 to December 2022,the medical records of the application of Guizhi plus Houpu Xingzi decoction in the outpatient department of Professor Li Weiwei,the supervisor,were collected,further screened according to the inclusion and exclusion criteria,formed data sources,and then standardized and cleaned the data to establish the application data warehouse of Guizhi plus Hopu Xingzi decoction."Traditional Chinese Medicine Inheritance Computing Platform(V3.0)" software was used to input data,clinical medical case analysis and data mining technology was used to make statistics on general information,diseases,symptoms and signs,and drugs,and data mining between symptoms and signs and between drugs,and the results were analyzed and discussed in combination with the basic theories of traditional Chinese medicine.To summarize the prescription and syndrome rule of Professor Li Weiwei in treating pediatric pulmonary diseases with this prescription.Results:1.General information of patients: Among 387 medical cases,243 were males and 144 were females,with a male to female ratio of 1.7:1.Preschool children(3-6 years old)had the highest prevalence,accounting for 54.78%,respectively.Spring and autumn accounted for 27.90% and 40.83%,respectively.2.Diagnosis and statistics of diseases: There are 5 kinds of lung diseases in TCM,with cough and cold as the most cases,accounted for 75.97% and21.19%.There are 5 kinds of TCM syndrome,among which the most cases are wind cold attacking lung syndrome and wind pathogenic syndrome,accounting for 75.97% and 15.50% respectively.In Western medicine,there were 7 kinds of respiratory diseases,with acute bronchitis,bronchitis and acute upper respiratory tract infection accounting for 57.25%,29.01% and7.63%,respectively.3.Four diagnosis of TCM and symptoms and signs: There were 74 kinds of symptoms and signs in the statistics of symptoms and signs,among which12 symptoms and signs were the main symptoms with high frequency,including coarse breath sounds in both lungs,cough,runny nose,nasal congestion,urination,enlargement of tonsils,no hearing of dry and wet rales,defecation,sleeplessness,sputum,sweating,and no fever.There were 3species of tongue texture,8 species of tongue coating,and 13 species of pulse pattern.The most frequent pulse pattern of tongue coating was light red tongue,white tongue coating,and slow pulse.The results of association rules showed that the rules with high correlation of symptoms and signs were:Sputum => cough(60.72%,99%),sweating => bilateral lung breath sounds coarse(60.47%,97%),tonsil enlargement => bilateral lung breath sounds coarse(72.35%,95%),nasal congestion => bilateral lung breath sounds coarse(79.07%,93%),cough => bilateral lung breath sounds coarse(82.17%,92%),runny nose => bilateral lung breath sound coarse(81.40%,92%),no heard dry and wet rales => bilateral lung breath sound coarse(66.41%,92%),tonsil enlargement => runny nose(68.73%,90%),tonsil enlargement =>cough(68.48%,90%),nasal obstruction + tonsil enlargement => bilateral lung breath sound coarse(62.53%,95%),running-nose + nasal obstruction =>bilateral lung breath sound coarse(70.28%,92%),etc.According to cluster analysis,6 symptom core combinations were obtained.4.Traditional Chinese medicine analysis: A total of 59 TCM were used in387 cases,with an average of 13.43 TCM in each case,among which 9 TCM,cassia twig,white peony root,roasted licorice,jujube,bitter almond,magnolia officinalis,yam,ginger and malt,accounted for the highest frequency and were the main medicines.In the statistical results of four gases of drugs,warm drugs were the most frequent.The most frequent drug was sweet medicine.In the statistical results of drug tropism,the most frequent was the drug tropism of spleen meridian.In the results of drug efficacy statistics,the high frequency of drugs were tonic drugs,surface drugs,and digestion drugs.The results of association rules showed that the correlation between the original prescription drugs was extremely high,and the rules with high correlation between the modified drugs were as follows: Malt => yam(93.02%,99%),yam => malt(93.02%,98%),jujube + shoot dried => yam(59.95%,100%),malt + hawthorn => yam(90.44%,100%),fried licorices +malt + acorus calamus => yam(89.41%,100%),etc.According to cluster analysis,6 core drug combinations were obtained.Conclusion:1.Professor Li Weiwei used Guizhi plus Houpu Xingzi decoction in the treatment of pediatric pulmonary diseases in children under 5 years of age,and it was more common in the spring and autumn when cold and hot alternate.In pulmonary diseases,it is mostly used for cough,and the syndrome type of pulmonary diseases is wind-cold syndrome.The main points of syndrome differentiation were cough,phlegm,nasal congestion,runny nose,sweating,pink tongue,white coat,and slow pulse.The main core points of related symptoms and signs are "positive deficiency","phlegm stagnation","external sensation","qi inverse" and "phlegm obstruction".It is concluded that the etiology and pathogenesis are positive deficiency,phlegm stagnation,wind and cold cluster surface invasion of the lung,lung loss,lung qi inverse,qi is phlegm obstruction.The location of the disease is in the lung,involving the spleen and kidney.The treatment is to regulate the Ying-wei,solve the external theory,reduce qi and dissipate phlegm,relieve cough and asthma.Guizhi plus Houpu Xingzi Decoction has a stable drug structure.Cassia twig,paeonia lactiflora,processed licorice,Magnolia officinalis and almond are the core drugs,which are most compatible with tonic and digestive drugs.Based on the support and confidence of drug association,the correlation between drugs is deeply explored,which reflects the prescription idea of the tutor and provides an objective basis for clinical drug compatibility.2.Professor Li Weiwei is good at treating pediatric pulmonary diseases such as cough with Guizhi plus Houpu Xingzi decoction.Based on the concept of "neutral children’s safety" and the idea of "one breath flow",Professor Li Weiwei combined with traditional Chinese medicine classic prescriptions to make the prescription infantilizing and syndrome differentiation.It is believed that children’s deficiency is often responsible for the deficiency of lung and spleen in children,the lungs are delicate,the lung main surface,and the skin and hair,lung qi deficiency is not stable and susceptible to external pathogens,spleen deficiency is lost in health,and children’s diet can not be self-controlled,easy to eat stagnation and phlegm and drink,every case of wind and cold pathogenic surface attack lung,Lung loss of propaganda and descent,lung qi on the contrary,qi for phlegm block,then cough and asthma.The treatment is based on Guizhi plus Houpu Xingzi decoction,combined with supplementing deficiency,invigorating spleen and dissipation of food and other drugs,internal and external treatment,relieving the surface theory,gently regulating and supplementing,regulating qi,and treating according to the syndrome.
